0476389380 2013-04-23 Production Wells No Violations FOLLOW UP TO NOV # 1366989100 This is a pumping unit wellhead. There was no identification sign at the wellhead. According to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owners name, county, township, well name and number, permit number for the well, and emergency contact number. There is a t-fitting at the ground surface. From the cap on this fitting is 1-7/8 inch production tubing rises to another t-fitting. On top of this fitting is the stuffing box assembly for this pumping unit. The tank battery for this well and the Roush #1 (34-053-20427-00-00) are in a common containment area for two other different leases, the Shuler and Coen. There are two 100-barrel tanks and one 150-barrel tank for the Roush lease wells. Lease names are written in the sides of the tanks. There is one 1100-gallon poly tank, one liquid/gas separator and one small knockout tank also in this containment area. The containment area needs work on the dikes surrounding this area. There is also another tank battery for just the Roush wells closer to their wellheads (GPS 38.96265/-82.11591). This battery consists of two 100-barrel steel tanks, one tall liquid/gas separator, and one small knockout tank. As with the other tank battery this one needs higher dikes around the containment area.

1770292455 2017-05-03 Production Wells No Violations On the day of my inspection, I am accompanied by inspector Matt Fulk. Inspection revealed an oil and gas well with 4.5 inch diameter casing, 1.5 inch diameter tubing and a pumping unit with a gas motor.

1803633812 2007-06-26 Production Wells No Violations Well changed ownership on 6-28-2006. Since that time a logging operation has prevented the new owners from access to the well. Lines have been damaged due to the logging operation. The well does not have an id. Pump jack does have a motor. Well has a gas leak. Barry Bowers told me that they have some work to do with these new wells. I am passing the old NOV that was to the old owner. I will follow up with Bowers later in the year.
