0124687598 2009-06-08 Production Wells No Violations Oil spill follow up. Meet with Pete and Paul on site. They are going to move tank across creek near well. We discussed the 50 foot spacing requirements between the tank and the well. They are leaving the mechanical separator and gas meter in the original location. We looked at the contamination on the stream bank at the 1st underflow dam (nearest to tank dike). They informed me that they have hauled 2 tons of top soil into the Workman yard (Complaint 6-2-09) and filled in all low areas and have seeded and mulched the area.

The siphon dam is effective, it has contained the oil. 1st Siphon dam is constructed of soil and straw bales ( 9 bales) and three 6 PVC pipes. Altier Brothers water truck is on 1st location. No one is at 1st siphon dam location. 2nd Siphon dam GPS N 38.94925 / W082.15058. 2nd dam constructed of soil and four 4 PVC pipes. They are rebuilding the tank dike GPS N 38.94949 / W 082.15088. Estimated 5 bbls oil in creek and 30 bbls oil contained in dike. Pumped oil in dike back into tank and repaired tank with a plug.

1243967354 2009-05-16 Production Wells Violations Noted A complaint was received by the Division of Mineral Resources Management alleging that an oil spill entered a creek located on Turkey Run Road in Chesire, Ohio. Arrived at the downstream location and found that the well owner, Altheirs Oil Inc., has contained the spill. At this location, a siphoning dam was observed and a absorbent boom deployed behind this installment. A water truck was at this location siphoning out crude oil from Turkey Run Creek, a tributary of Kyger Creek, which drains into the Ohio River. An inspection upstream from this location revealed that the source of the spill was from a broken load line on a 100 bbl tank with no knock-out. Tire tracks observed near the location revealed that the line may have been clipped with a vehicle. The vegetation inside of dike, contained an oil stain approximately 6 to 1, on average, below the top of dike. Oil stain observed at top of dike near the load line connection to tank, which may have resulted from the initial break of load line. At this time, it is believed that the oil appeared to have leached into Turkey Run Creek from the bottom/side of earthen dike. Note: Tank is located approximately 50 away from creek. Production equipment at this location consists of (1)100 bbl tank with no knock-out and separator. Altheirs Oil Inc., Representative, Mr. Dale Compton, stated that 2.5 to 3 of oil had been released, but majority contained inside the dike. All fluid had been pumped back into the tank. A second siphoning dam was also installed and located approximately 50 downstream from the tank location. The overall length of impacted stream is approximately 0.75 mile. The estimated release of crude oil at this time is between 3 to 5 bbls.
1244139329 2009-05-18 Production Wells No Violations A follow-up inspection to crude oil spill revealed that progress has been made on cleanup. 500 downstream from source the creek appeared to be clear. 0.25 mile downstream an oil sheen was observed. 0.5 mile downstream oil appears to be breaking down to include small pockets of oil. As previously noted, the creek contains is vegetated in areas downstream from tank and upstream from siphoning dam. At the time of the inspection, low flow was observed. Also noted, mulch and pads have been placed inside dike to cleanup pooled oil.

1262789147 2009-06-03 Production Wells No Violations Received a Complaint message from Doris Workman 551 Turkey Run Road. 740-367-0291, on 6-2-09 at 7:30 pm. Message was received at 6:30am on 6-3-09. A return call to Doris was made on 6-3-09 at 7:10am. Complaint was oil in front yard. Inspected the site at 8:29am on 6-3-09 and observed that the 2nd siphon dam near the culvert has been washed out and is not functional. 6 PVC pipes from the siphon dam are clogged with grass cuttings. This clog has contributed to the siphon dam failure. Absorb pads and booms were washed away and deposited along the bank of the stream which runs through Doris Workmans front yard. 8 low areas retaining 2 to 3 of water with a sheen on top of the water. Total area for all 8 is 30 by 30 GPS N38.94273 /W082.14054. Absorb pads and booms are along the stream bank. I did not see crude oil only a sheen. The brown film did not stick to my finger. I noticed no oil smell. Pictures were taken. The clean up crew was notified and met me on location to discuss clean up efforts. Mike Sharron OEPA arrived on site and we were able to verify that the sheen is oil and must be cleaned up.

1511268094 2017-11-20 Exempt Well Pre Transfer No Violations On the day of this inspection, I found an oil and gas well consisting of 4.5 inch diameter casing, 1.5 inch diameter tubing with a pumping unit equipped with an electric motor. Tank battery consisted of one 100 barrel steel production tank set in an earthen dike. Identification was legible and located at the tank battery. Landowner Jim Ward met with me at his house and showed me the gas connection from the gas well to his property. Gas line connected to the property consisted of a gate valve and low pressure regulator and was working properly during my inspection.
