1033755843 2004-08-02 Production Wells No Violations Looked for 1.5 hours for well using GPS - could not locate site -located on a strip mine spoil and may have been covered up.
1143786568 2013-04-25 Production Wells Violations Noted This is a pumping unit wellhead. There is a t-fitting at the surface with two 2/3/8 inch pipes coming out both sides of the fitting. One of the pipes ends in a ball valve and the other starts the production lines from the wellhead. A 1-1/16 inch production tubing rises out the cap on this fitting ending in another t-fitting. Coming out the sides of this fitting is a 2-3/8 inch pipe connected to the production plumbing and a 1-3/8 inch tube is coming out the other side ending in a ball valve. A 2-1/2 inch pipe rises out of the top of this fitting into the stuffing box assembly for this pumping unit. The tank battery consists of one 100-barrel steel tank, one tall liquid/gas separator, and one small knockout tank in a containment area. There is a hole on the south side of this area making this containment ineffectual. The operator shall plug the hole to improve containment of any fugitive liquid releases according to 1501:NONE(B)/1509:22(C).
