| 0769022147 | 2013-04-25 | Production Wells | Violations Noted | This is a gas wellhead. The wellhead is missing an identification sign. According to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owners name, county, township, well name and number, permit number for the well, and emergency contact number. There is a 7-1/8 inch t-fitting at the ground surface. Two 2-3/8 inch pipes are coming out the sides of this fitting with a bull plug at the end of one and a ball valve at the end of the other. A 3-9/16 inch casing is rising out the top of this fitting ending in another t-fitting. Two 2-1/2 inch pipes are coming out the sides of this fitting. A 1-1/16 inch production tubing is coming out the top of this fitting and ending in a master valve. At the top of the valve a 2-1/2 inch pipe is coming out. Eventually the rest of the plumbing is 2-7/16 inch pipes finally plunging into the ground. The tank battery consists of one 100-barrel steel tank, one tall liquid/gas separator, and one small knockout tank in a containment area. There is a hole on the south side of this area making this containment ineffectual. The operator shall plug the hole to improve containment of any fugitive liquid releases according to 1501:NONE(B)/1509:22(C). |
| 1282718494 | 2014-05-14 | Production Wells | Violations Noted | An 1 1/2 orange plastic line was found cracked and leaking a small amount of gas. All faulty equipment shall be repaired or replaced. Any contaminated soils shall be removed and steps shall be taken to encourage vegetative cover. |
