1230302552 2014-04-24 Plug / Plug Back No Violations Upon inspection, it was observed that this site is still largely bare. The pits were filled in. Surface casing was sticking approximately 3 out of the ground and cement could be seen about 3 down. I contacted Tim Wyant of Wyant Exploration Co. to discuss the restoration of this site, and he informed me that AEP waived the final restoration because they have plans to expand their landfill into the area where these wells were plugged. We do not have the landowner waiver on file. I will contact Tim Wyant again to see if he has a copy of this waiver. The restoration will not be passed until the waiver is approved.

1366380623 2013-04-17 Production Wells No Violations This is a Pumping Unit Wellhead. Cap on surface casing was at ground surface. Could not determine surface casing diameter. A 4-inch casing rises above this cap ending in a t-fitting. Two 2-1/4 inch pipes are attached to the side fittings. A 1-7/8 inch production tubing is rising out of this fitting to another t-fitting above. On top of this fitting is a stuffing box assembly for the Pump Units sucker rods. Tank battery for this well consists of two 100-barrel tanks in separate containment areas, and two tall liquid/gas separators.
1503202832 2015-06-16 Final Restoration No Violations On my inspection I found that the location had been restored according to 1509.072. I passed the final restoration at this time.
