1303838663 2011-04-26 Production Wells Violations Noted Received call from OEPA at 7:29am. Well leaking gas and fluids at 343 Eakman Road Patriot, Ohio. I arrived on location at 9:28am and located the well. Wellhead has scald/contamination area 18 feet by 20 feet. Vegetation is scalded and tinted black, with sheen on standing dead vegetation. Photos were taken. A blank Enervest identification was attached to the mechanical separator. Well must have identification(1509:9-9-05(A)(10): Well owner, lease name, well number, permit number, county, emergency telephone number. Well is configured for plunger lift. Tubing has two holes located between the wellhead and the master valve. Brine is leaking out of the holes in the tubing. No LELs were recorded on meter during inspection. A High End Quantab was placed on wellhead and was maxed out over 6731 ppm Cl. A second High End Quantab was placed in pool of fluid at base of wellhead it maxed out over 6731 ppm Cl. 2100 gallon plastic tank has a dike around it that is full of water. Replace defective tubing joint or repair holes in tubing. Remove contaminated soil and take to EPA approved land fill. Restore vegetative cover to well site.
