0037698645 2014-03-28 Production Wells No Violations My inspection found the wellhead has been rebuilt with new valves connecting the well to the flow line. The brush inside the tank battery has been removed. The mechanical separator has been removed and the flow line has been plumbed into the production tank. There still is no identification on location for the well. The dike needs to be rebuilt if the well is able to be produced. The company plans to swab the well in May to determine if the well is able to be placed back into production.
0132851368 2018-07-05 UrbanProduction Wells Violations Noted I conducted an inspection on this well site. At the time of this inspection, the tank battery consisted of one 100 BBL tank and a 50 BBL poly tank. The 50 BBL poly tank was crushed on top. The oil level in the tank was at capacity and some of it had leaked out into the diked tank battery. The free oil was located along the south length of the inside of the dike. The measured area of the free oil was 4 foot by 26 foot. Signage was located at the tank battery. I contacted Dry Hole Producing and spoke with Eric Lindburg at 330-720-8185 to discuss removal of the free oil in the dike, and replacement of the 50 BBL poly tank. Mr. Lindburg indicated that the company would be onsite today to start the cleanup.
0836619631 2014-02-14 Production Wells Violations Noted This inspection is a follow up to compliance notice inspection number 1363696872. My inspection found no changes to the well since my last inspection. The well is not in production. I have spoken with Eric Lindberg on numerous occasions about the status of the well. Eric and his partner J. Adams have assessed the well last week and will be contacting me about the companies plans for the well.

1363696872 2013-03-19 Production Wells Violations Noted My inspection found the wellhead to be disconnected from the flow line. The lubricator on the wellhead was removed leaving the demco valve exposed. The demco was closed. The casing line valve was closed that lead to a house line. The tank battery is surrounded by a chain link fence. The tank battery has no dike and is over grown with 2 and 3 tree saplings. The tank battery consists of a 100 BBL production tank and a mechanical separator. Inside the fence there is debris, garbage, and a equipment fuel tank. There is no identification sign on the tank battery. The production records show that the well has not produced since 2008. The lease road is in stable condition. Spoke with Eric from Dry Hole Production 330-720-8185, he stated that he would file for the plugging permit.
1395941828 2014-03-27 Production Wells No Violations I spoke with Eric Lindberg from Dry Hole Producing about the issues of the well. He stated they just spent the past week putting the wellhead back together and running new lines inside the tank battery. He stated once the ground dries up he will get a service rig in to service the well. If the well does not produce anything he will go ahead with plugging the well. I will go out to the location to inspect the progress of the work on the well.

1417103253 2018-07-13 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 132851368. At the time of this inspection, the 50 BBL tank has been cleaned out and the fluid removed. The free oil/water within the dike has been removed and peat has been added to absorb what is left.

Dry Hole Producing still needs to: 1) Remove the impacted soil/stone from the battery and dispose of it properly. Add new soil and stone. 2) Remove the 50 BBL poly tank from the site.

1470057712 2016-07-18 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection on this well. The well consisted of the 8 5/8 inch casing with tubing and valve with a Swage, which had then been plumbed into a two inch flow line. The well is being used as a swab well. The valve on the tubing was closed. The tank battery consisted of one, 100 BBL production tank and a concrete brine tank. The tank battery was diked but the concrete brine tank was outside of the dike. Inside the dike there is debris and an equipment fuel tank. Part of the chain link fence has been removed and not reinstalled. Proper identification was located at the well and tank battery. The last recorded production for this well was in 2015 with 38 BBL of oil. The lease road was in stable condition. I spoke with Eric Lindberg from Dry Hole Production, about removing the debris and fuel tank from the tank battery, reinstalling the chain link fence, and addressing the brine tank. He indicated that the concrete brine tank is going to be cleaned and removed and the fence reinstalled once that work was completed. He also stated that the debris and fuel tank will be removed from the tank battery as well. The previous compliance issues of required production records, signage and putting the well back into production has been remedied. Compliance notice can be closed.
1535118021 2018-08-24 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 132851368. At the time of this inspection, the 50 BBL tank has been cleaned out and the fluid removed. Some stained soil still remains within the diked battery and needs removed. The dike around the tank battery has been reworked. The 50 BBL poly tank still needs removed. I contacted Eric Lindbergh of Dry Hole Producing regarding the removal of the 50 BBL tank and the rest of the impacted soil. Mr. Lindbergh stated that he would be here the week of August 27th, 2018 to remove the tank and conduct final soil remediation.

Dry Hole Producing still needs to: 1) Remove the impacted soil/stone from the battery and dispose of it properly. Add new soil and stone. 2) Remove the 50 BBL poly tank from the site.

1545317186 2018-12-20 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 132851368. At the time of this inspection, the northwest corner of the inside of the tank battery has had the impacted soil and stone removed. There remains a 100 BBL tank with legible identification and a reconstructed dike. All required issues regarding this compliance notice have been addressed. Compliance notice closed.
