Prior to setting the Big Lime plug, the 4.5 inch casing was ripped at 2621 feet and removed from the hole. Nicks Well Plugging (NWP) broke circulation and pumped 88 BBLs of fresh water to purge out the static oil and clean out the hole before running the 2 3/8 inch tubing back into the hole, to set the Big Lime plug. The Big Lime plug was set from 2550 feet to 2250 feet using the 2 3/8 inch tubing. NWP removed 61 joints of tubing to set the surface plug. NWP broke circulation and circulated 3 BBLs of fresh water prior to setting the surface plug. Surface plug was set from 520 feet to 320 feet using 2 3/8 inch tubing. 6 more joints of tubing were pulled to set the top plug from 200 feet to surface. Cement was pumped until it reached the surface and topped off with any remaining cement. |

The well is a 4.5 inch casing and tubing with a gate valve and bull plug on top. The pumpjack and rods have been removed. The valves on the wellhead are closed. The tank battery is over grown with weeds and tree saplings. The tank battery consists of two, 100 BBL production tanks and a buried concrete brine tank. The separator appears to have been removed from the location. The identification on the tanks was illegible. The access was stable. Recorded production has been zero since 1996. I contacted Richard Campola from Select Energy Investments about the plugging of this well. He indicated that application had been made to ODNR in order to obtain a plugging permit. The well is scheduled to be plugged this calendar year. |
1500390090 | 2017-07-17 | Plug / Plug Back | No Violations | I was on location to witness the plugging of this well. Nicks Well Plugging is the plugging contractor for Select Energy Investments, Inc. The pumpjack and rods had been removed from the well prior to the start of the plug job. It was determined that the 4. 5 inch casing had a hole in it so the 2 3/8 inch tubing was to remain in the well casing for the setting of the bottom plug. Nicks rigged up and circulated through the 2 3/8 inch tubing until the oil was cleaned from the casing. The bottom plug was set from 4042 to 3642 with 30 sacks of Class A cement. The tubing was then pulled from the well prior to ripping the 4.5 inch casing. The casing was ripped at 2621 and completely removed from the well. Well was then shut in until next day. |

The separator appears to have been removed from the location. There is no identification on location. Production records show the well has not had production reported since 1996. I will contact the company about the well. The well shall be placed back into production in 90 days. If the well is not producible a plugging permit shall be obtained within 90 days and the well shall be plugged within 180 days of this compliance action. |
