0035345124 | 2012-05-10 | Production Wells | No Violations | My inspection today found no change in the status of the plunger lift well. The tubing has not been repaired. The flow line is disconnected from the wellhead. Minor odor of gas coming from the tubing. No visible venting of gas. Operator has removed the 210 barrel production tank that was leaking. Operator partially cleaned up paraffin and free crude oil. Remaining free crude is locked up in the gravel. Called operator and left a message to contact me in regards to the status of the well and getting it back into production. |
0429214607 | 2018-03-02 | Production Wells | No Violations | I inspected this well pursuant to Chiefs Order #2017-17 issued for lack of insurance. The well and associated battery are located on an otherwise undeveloped, rural parcel At the time of this inspection, I noted the following: a wellhead with plunger-lift system in a clearing within a wooded and grassy area. The system was idle. The flow-line connections were closed. The access road to the well was heavily overgrown and rutted. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator, that is shared with the Gibbs #1 (#2-0917). The tank was empty, and the fittings and connections were in disrepair. The secondary containment was mostly overgrown with vegetation and covered with several inches of accumulated snow. There were no signs that the well was being produced. |
0801023471 | 2016-02-04 | Production Wells | No Violations | My inspection found the company has recently serviced the well. The valves on the wellhead are currently closed. The brush has been removed from the tank battery. The production tank has a oil coating on the side from swabbing the well into the tank. I contacted the well tender for the company about cleaning up the oil on the tank and updating the identification sign. He will also contact me if the wells get put back into production. |
0926107201 | 2013-08-21 | Production Wells | No Violations | Damaged tubing has been replaced. I new demco valve has been installed. |
1044475280 | 2013-08-21 | Production Wells | No Violations | My inspection found the wellhead was plumbed up from the tubing to the demco valve. The demco was closed and the rest of the wellhead had been disassembled. The tank battery consists of a 100 BBL production tank and a mechanical separator. There was no identification sign on location. There has been no production reported since 2010. I will contact the company about the issues. |
1408023163 | 2014-08-14 | Production Wells | No Violations | My inspection found no changes since my last inspection. The tank battery is over grown with vegetation and needs to be cleared out. The company is currently under chiefs order to stop operating in the state until they obtain the proper insurance. I will contact the company about the issues. |
1989964865 | 2011-11-10 | Production Wells | Violations Noted | Well is a rabbit well. Tubing below the Demco valve has a hole in it, the hole is half of the diameter of the tubing. Valves on the 4.5 inch casing are closed. Tubing was static only a slight odor of gas coming from the tubing. Spoke to Bob Barr on 11/14/2011, the well is shut in because they do not have the correct meter to flow gas into East Ohio lines. New meter is under construction. Tubing in this well has a salt ring, they have attempted to pull the tubing. They have put fresh water down the back side to attempt to free tubing. Tank battery is over grown with vegetation. The bottom of the 210Bbl production tank is leaking. There was standing free oil on the floor of the battery below the plumbing on the west side of tank. 100Bbl tank had oil staining on the floor of the battery under the valve. No oil outside of the dike. Will attempt to meet with work crew at another well to discuss needed repairs. Informed Bob of need repairs. Well needs the following: 1. Tubing repaired 2. Free oil and oil staining removed from tank battery 3. Vegetation cleared from tank battery. 4. Tanks repaired as need to contain oil and brine 5. Well put back into production or put into temporary inactive status 6. Identification sign posted at the tank battery and the wellhead. Picture of the well and tank battery taken this day. |
