The tank battery consists of a 100 BBL production tank and a mechanical separator. There was proper identification on the wellhead. The tank battery did not have the proper identification on it. There has been no production reported since 2008. The tank battery is being shared by permit number 2-0916. The company is currently under a chiefs order and not aloud to operate in the state. I will contact the company about the issues. |

The brush has been removed from the tank battery. The production tank has a oil coating on the side from swabbing the well into the tank. I contacted the well tender for the company about cleaning up the oil on the tank and updating the identification sign. He stated he has to get a hole in the flow line fixed before the well can be put back into production and will keep me informed on when the repairs will be made. |
| 1321282880 | 2011-11-10 | Production Wells | No Violations | I received a call from Soil and Water. They received a call from Mrs. Gibbs stating that she hasnt received royalty checks in some time. This is a rabbit well and it is shut in. The Demco valve and valves on the 4.5 casing were closed. There is an identification sign was posted at the wellhead. No production has been reported on this well since 2008. Permit #20916 is also produced into the same tank battery. Tank battery consists of one 210 barrel tank, one 100 barrel tank and a mechincal separator. Both the 210 barrel tank and the 100 barrel tank are leaking. There was free oil at the bottom of both tanks. There was oil staining on the tank battery pad at the west side of the 210 barrel tank. An area approximately 4 X 15 was affected. The tank battery was over grown. Bob Barr was called and a message was left to contact me in regards to the well. He called on 11/14/2011 and stated that the well was shut in by East Ohio because they have the wrong type of meter. The line pressure is at 500 PSI and they are not set up to produce into that line pressure. The oil spill and leaking tanks need repaired and all impacted soil needs removed and disposed of properly. Applications for temporary inactive status for permits #20916 and #20917 need submitted. Compliance notice will be attached to permit 20916. |

The well and associated battery are located on an otherwise undeveloped, rural parcel At the time of this inspection, I noted the following: a wellhead with plunger-lift system in a small clearing in a wooded area. The system was idle. The flow-line connections were closed. The access road to the well was heavily overgrown and rutted. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator, that is shared with the Gibbs #1 (#2-0916). The tank was empty, and the fittings and connections were in disrepair. The secondary containment was mostly overgrown with vegetation and covered with several inches of accumulated snow. There were no signs that the well was being produced. |
