0037264359 2015-12-01 Production Wells No Violations My inspection found the company has recently brought in a swab rig and removed a 100 BBL full of oil after the maintenance was completed. The well has pressured up to over 750 psi. The well tender is currently replacing the flow line. Dominion currently has the sales meter shut in so the company is unable to sell gas. Once Dominion allows the company to sell the well will be put back into production. The company that owns the property has stacked lime stone up next to the tank battery and well and it is beginning to spill over the dike and into the tank battery. The dike wall on the West side of the tank battery has been removed by the property owners equipment. I have contacted Ralph Kyanko for Kokosing Materials and spoke with him about repairing the dike. We are to meet on location this month to discuss issues about the well.
0095200150 2011-03-08 Production Wells No Violations 4.5 inch casing is under water. Valve is at the surface of the water and was visible. The valve was closed. Operator has attached a 2 inch pipe to the top of the valve. Gas was bubbling up around the casing. Bubbling was taking place approximately 2 feet around the wellhead under water. Tested water for Cl-, no Cl- noted. Well head was not connected to the tank battery in noticeable manner. Black plastic pipe was coiled at the tank battery location. Separator valves were closed.
0136459528 2009-12-02 Production Wells No Violations Tank battery has been roughed in, separator and 100bbl tank is on site. ID sign is not posted on the battery or separator. Well head is below grade, a pipe with a valve extends out of the ground with a traffic cone on it. Gas is bubbling up around the location of the well head. There is approximately 6 inches of water on top of the ground where the well head is. Pulled water sample and test for CL-, 1.8 on the low range CL tab, approximately 65ppm of CL, water is fresh water. 2 inch black plastic tubing was on location in a roll. Well is not plumbed into the tank battery at this time. Oil covered pipes where laying next to the tank battery, pipes appeared to be from the pump jack, that had been on the well.

Well is leaking gas to the surface, there was bubbling through the water standing around the well. Valve on the well was closed. Pictures taken during inspection. Black plastic pipe was coiled and stocked next to the separator. Chart has rusted off of the separator and was in the meter box. Pipe coming out of the ground on the west side of the separator was shut off.

Meter box has been attached to the separator.

The valve is open on the flow line leading into the separator. The valve on the sales line leading out of the separator is open. The valves on the meter run by the entry to the location are open. No other changes to the location since my last inspection.

A dike was established around the tank battery. An identification sign was on the production tank. The tank ladder was no longer laying on the ground and was attached to the production tank. The spacing issues have not been corrected. The flow line still needs to be buried.

Operator may be in discussion with Broad Street Energy and Northwood Energy to produce well into their sales line. Called Steve Chomos from Broad Street, there was a discussion no agreement was made. Called Northwood Energy, left message. Doug stated that he has contacted Jean Bruner at 14864 Mayfield Road in regards to domestic supply. Jean has not been receiving domestic supply from well.

Sales line has not been connected to the separator, connection threads have been installed on the north end of the sales line. Production tank has been connected to the separator. Spacing between the separator and the production tank has been corrected. South end of the sales line has not been connected to the tap. Sales line is above ground within 50 of the tap. Chart is not connected to the separator and 1/4 lines from the meter box to the separator are disconnected at the separator. Operator has stated that he is going to produce the well on a Con-Coil and I called Bob on Oct 3, 2011 at 12:32pm leaving a message requesting information on when that was going to take place. Have not received return call or information on whenCon-Coil is to be installed. A well leaking gases into the atmosphere is in non-compliance with 1509.12. Operator needs to take corrective actions to repair leak. The well has had no reported production since 2000.

There is no identification on the well. The lease road is stable. Production has not been reported in 2011 and 2012. There is a trencher that has been sitting on the location for a year. I spoke with Jamie Cambell about having the flow line buried, the trencher removed from the location, getting an ID sign on the well, and cleaning up the location. He will find out about the production and report back to me.

There is no identification on the well. The lease road is stable. Production has not been reported in 2011 and 2012. Production was also not reported for the years 2001 to 2010. There is a trencher that has been sitting on the location for a year. Myself and my supervisor Tom Hill have made numerous attempts to get the issues remedied. The flow line needs buried. The spacing between the wellhead and the separator needs to be made and the spacing between the separator and the production tank needs corrected. The proper containment around the tank battery needs to be established. The trencher cannot be stored on the location. The proper identification needs to be visible on the tank battery. Production needs to be reported for 2011 and 2012. These actions need to be performed within 30 days of receiving this notice.
1389275375 2014-01-09 Production Wells No Violations My inspection found the ladder for the production tank has been knocked off of the tank. I will contact the company about having the ladder reattached to the production tank. The well was producing while I was on location. It has a production cycle of 3 minutes every 8 hours. There has been no other changes since my last inspection.
1393929876 2011-06-24 Production Wells No Violations On site with Jim Bailey who is attempting to flow the well into the production tank. After discussing the procedure with Supervisor Rick Simmers, Rick and I were in agreement, that this method of production is not approved. Blowing a well down and flowing it into a production tank using a flexible line is not an standard method of producing a well and is not allowed. Well is not in currently in production and equipment spacing is not correct. The distance between the separator and well head is 48. The distance between separator and production tank is 5.4. The distance between separator and wellhead must be 50 feetand the distance between a mechincal separator and production tank must be 50 feet. Per Jim, the sales line runs north then east towards the railroad bed and connects to a Broad Street sales line. Also, there is no agreement between companies to sell into their line. The operator is planning on running a new sales line south and connecting it to an existing East Ohio line.

Geneva pipe installed tap, check valve and valve this day. Jim was informed if he starts work on the Kokosing property the Sheriff would be contacted to remove them from property for trespassing. Deputy Sheriff on site in the after noon. It was mutually decided that the matter of the validity of the lease is a civil matter. Decision to continue work is up to the company. Jim needed parts to start work on the sales line will start work next week.
