1390974426 2013-10-21 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The tank battery consists of a 210 BBL production tank, a 210 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The well has not shown production since 2000. I was informed that the casing was collapsed. The 210 BBL tank has rusted through on the bottom and there is oil saturated stone around the base of the tank. I notified Jim Dietrich the well tender for Enervest that the tank needs to be removed and the oil contaminated stone needs to be removed. I notified Mr. McDaniel a Supervisor for Enervest that the well needs to be produced or plugged. If the casing is in fact collapsed the well needs repaired or plugged as soon as possible.
1412689798 2014-10-06 Final Restoration No Violations My inspection found the location still needs graded, seeded, and strawed. Once vegetation is established and the waiver is submitted I will be able to pass final restoration. I contacted Kyle Ahbel from Enervest about the restoration. He stated the well is next on the list to be completed.

1992239354 2016-05-05 Final Restoration No Violations My inspection found vegetation has been established on the location. The trees that were affected during the plugging operation have been removed from the location. A waiver has been submitted for the flow line and lease road. Final restoration can be passed.
