0958774980 2017-07-10 Production Wells Violations Noted During this inspection, the well was equipped with a plunger lift assembly and valves on the wellhead were open. There was a domestic line with a meter, drip bottle and two regulators plumbed off of the 4.5 inch casing with an open valve. The tank battery was consisted of one 210 BBL production tank, one 50 BBL steel brine tank and a mechanical separator. Valves on the separator were closed and chart box has been removed. Review of the production records showed that they were up to date. The 210 BBL production had holes in the side of the tank and some fluid had seeped out and stained an area measured 5X7 feet around the base of the 210. The tank battery was diked, but was overgrown with small brush. Signage was located at the tank battery and the lease access was stable. The following items need addressed:

1) Clean-up and remove all of the oil impacted soil within the tank battery and properly dispose of the impacted soil. 2) Remove and replace the leaking tank. 3) Remove and maintain vegetation from the tank battery.

1237831948 2009-03-23 Production Wells No Violations Well is a rabbit well. Tank battery has one 50BBL tank and one 210. Well and tank battery is fenced. Well and tank battery are in good repair.
1501001008 2017-07-25 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 958774980. The 210 BBL production had been removed, however the impacted area had not been cleaned up. Some of the brush had been removed to allow for the 210 BBL tank to be taken out, but the rest of the tank battery was still overgrown. The following items still need addressed:

1) Clean-up and remove all of the oil impacted soil within the tank battery and properly dispose of the impacted soil. 2) Remove and maintain vegetation from the tank battery.

1597084204 2015-02-20 Production Wells No Violations My inspection found the well equipped with a plunger lift unit. The tank battery was surrounded by a chain link fence and a proper dike. The tank battery consists of 210 BBL production tank, a 50 BBL brine tank, and a mechanical separator. The proper identification sign is located on the tank battery. The lease road is in stable condition and is being used for residential purposes.
1851913964 2017-06-05 Production Wells No Violations I conducted an inspection of this well at the request of the Middlefield Fire Department. The fire department responded to a call from the landowner regarding the leaking 210 BBL production tank. The well was equipped with a plunger lift assembly and valves on the wellhead were open. There was a domestic line with a meter, drip bottle and two regulators plumbed off of the 4.5 inch casing with an open valve. The tank battery was consisted of one 210 BBL production tank, one 50 BBL steel brine tank and a mechanical separator. Valves on the separator were closed and chart box removed, however production records for this well were up to date. The 210 BBL production had holes in the side of the tank and some fluid had seeped out and stained an area measured 5X7 feet around the base of the 210. The tank battery was diked, but was overgrown with small brush. Signage was located at the tank battery and the lease access was stable. I contacted John Raber of M&R Investments about removing the leaking 210 BBL tank and cleaning up the impacted area, along with conducting vegetative maintenance inside the tank battery.
1950157715 2017-11-02 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 958774980. Clean-up and removal of the oil impacted soil within the tank battery has been conducted. No other issues observed.
1973662230 2013-02-06 Production Wells No Violations My inspection found the well equipped with a plunger lift unit. The tank battery was surrounded by a chain link fence and a proper dike. The tank battery consists of 210 barrel steel production tank, a 50 barrel steel brine tank, and a mechanical separator. The separator did not have a production chart but the production records in our database are up to date. The identification sign is faded but you can still read the emergency contact information. I left a message with the well tender to update the identification sign. The lease road is in good condition and is being used for residential purposes.

The 210 BBL production had been removed, however the impacted area still needs cleaned up. Vegetative maintenance has been conducted inside and around the tank battery fence.
