Young trees and brush are growing up through and around the equipment. The tank battery is also overgrown and the backside of the dike has been breached. The well valves are closed and the flowline to the separator is closed. The chart on the gas meter shows no production. The DNR database shows no reported production since 2006 and records prior to 2006 shows declining production. The identification sign needs updated to show the current owner. The brush around the well head and tank battery needs to be removed. The well needs repaired as needed and put back into production or a plugging permit needs to be applied for and obtained by 02/05/2011. The well owner needs to comply with the terms of the plugging permit. Temporary Inactive status will not be approved for this well as there is a pending complaint associated with the status of the well. |

The excavator then opened up the area where the plugged well was located. Enervest Supervisor Ellewood McDaniels and I witnessed the plugged well was leaking inside the casing around the cement plug. I instructed him that the well would have to be drilled out and replugged. The chain link fence also needed removed and the tank battery reclaimed. |

I observed the wellhead to be fitted with a pumpjack which appears to be inactive. The wellhead is completely overgrown with weeds and dense foliage. The TB is comprised on one 210 barrel (BBL) production tank, one 110 BBL brine tank, and the remnants of a mechanical separator. The TB area in enclosed within a chainlink fence which is in a state of decay and disrepair. The interior of the TB is overgrown with foliage and small trees. This unit appears to have been idles for an extended amount of time. A follow-up with Tom Havranek of Enervest, indicates the unit is slated for plugging. |

In the area of the work pit, there was standing water with crude oil floating on it. I observed the pit; it appeared that there were bubbles coming up through the water. Kevin Perry contacted me that night; stated Enervest constructed a dike around the well. It appears the well is leaking and will need remedial action. Omega rigged up, broke circulation, and pumped 70 sacks of Class A cement to the surface. Cement returns were witnessed. After the tubing was removed from the surface, the hole was topped off with cement. |
