1262030421 2009-12-22 Production Wells No Violations On site to meet with representative from Howard Hanna, FHA loan was denied due to the spacing of the tank battery from the garage with an apartment in it on the property. Measured distance from production tank to the building, distance was 70. Separator was closer. Walked site with FHA requirements for spacing are different than ORC, I informed that I can only enforce ORC laws. There is a spacing issue and the company will be contacted. Called Don Craiger from David Hill, regarding the spacing issues. Don will meet me on site on December 28,2009.

The casing has three house lines plumbed into it. The well and the tank battery are in good repair and are surrounded by a wooden fence. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.

The well was fenced. The last reported production for this well was in 2015. The tank battery consisted of one 100 BBL production tank, one 100 BBL brine tank, a mechanical separator and a sales line. The valves on the separator were open and the chart showed production. The production tank vent stack was plumbed into a 55 gallon charcoal filter and then topped with a spark arrestor. The tank valves were equipped with flat plugs. The tank battery was diked and fenced but the fence had moved due to frost heave and no longer lined up with the man door opening, leaving the door unable to be closed and locked. The proper, legible signage was located at the tank battery location. The lease access was stable. I contacted John Suhar (740) 680-1483 from the David R. Hill Inc. and discussed the fence repair.

Took measurements with Don to work out how to move the battery to meet spacing. Tank battery was to be built into the side of the slope south side of the lease road. Tank battery was built at the end of the lease road. Operator has two 100BBL tanks and a mechanical separator in the battery. Tank will need to be moved west. Battery is in good repair. Dike is sufficient to prevent escape of fluids. Spoke to Dave Hill changes will be made immediately.
