0074756027 2013-07-25 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is closed. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank. There is no separator on location. The landowner informed me the separator was removed and the well has not produced since last year. Production records are up to date and show production for 2012. The lease road needs repaired. It is not passable with a vehicle to the well. The proper identification is on the tank battery. I spoke with Jim from Great Plains about the repairs and he stated that they were scheduled to be fixed.
0217929047 2018-10-17 Production Wells Violations Noted I conducted an inspection on this well. The well was equipped with a plunger lift assembly and not in production at the time of this inspection. Production records were last submitted in 2016 as zero. 2017 production records have not been submitted at the time of this inspection. The tank battery consisted of one 100 BBL production tank. The tank battery was diked. Signage was present at the tank battery but needed updated current owner information. The lease access was stable. The owner must place ID in compliance with OAC 1501-9:9-05(A)(10) by no later than 10/24/18 or be subject to additional enforcement. Contact Inspector Shannon Mullins at 330-413-5921.
1145467334 2016-03-22 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The valves in the wellhead are in the closed position. The annulus valve is closed. The well and the tank battery are in good repair. The tank battery consists of a 100 BBL production tank. The load line valves have plugs installed in them but are not locked. The heated separator is outside of the tank battery with the proper spacing. The flow line valve on the separator is in the closed position. The proper identification sign is installed on the tank battery. The lease road is in stable condition.
1211769237 2019-01-03 Production Wells Violations Noted I conducted an inspection on this well. At the time of this inspection, the well was equipped with a plunger lift assembly and not in production. The valves on the tubing and flow line were closed. Production records were last submitted in 2016 as zero. According to the state database, production records show no submission for 2017 for this well. The tank battery was comprised of one 100 BBL production tank. No other production equipment was present at the tank battery for this well. The tank battery was diked. Identification has not been installed at the well site location as required on compliance notice 217929047. The lease access was stable. I spoke with landowner Dan Lilly inquiring about this well. Mr. Lilly stated he has not received any royalty checks from the company for at least three years or more.

The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The Lilly 13B shall be plugged within 180 days.

1236359254 2009-03-06 Preliminary Restoration No Violations Site needs pit liner debris cleaned up. Repair seeding this spring maybe need if vegetation doesnt reestablish its self. Lease road needs dressed up. Called Tim from John D Oil informed him that road needs graded and pit liner debris needs picked up. Follow up restoration inspection will be need to make sure site is repaired.

The well was equipped with a plunger lift assembly and not in production at the time of this inspection. The tank battery was diked. Signage was present at the tank battery but needed to be updated to reflect current owner information. The lease access was stable. I contacted the David Oil Company, LLC about updating the signage.
1509375582 2017-10-17 Production Wells No Violations I conducted an inspection on this well in regards to complaint #5723. Landowner Dan Lilly was concerned about the well production status. At the time of this inspection, I met with landowner Dan Lilly on site and the well was equipped with a plunger lift assembly and the annulus and flow line valves were closed. Production records for this well were submitted in 2015 with 177 mcf of gas and in 2016 as zero. The tank battery consisted of one 100 BBL production tank. The tank battery was diked. Signage was present at the tank battery but needed to be updated to reflect current owner information. The lease access was stable. I contacted the David Oil Company, LLC at 440-951-1111 and spoke with a Jenny Corrao. She informed me that the field superintendent would be notified about updating the signage.

1804388419 2007-11-02 Drill / Deepen / Reopen No Violations On site for surface cement. Berea top 134 bottom 190. Surface set at 246 KB. 8.625 pipe new R-3 20 lb. Hole was 11 inches TD at 270. Operator pumped cement until it returned to surface, 15 BBL return of cement to surface. Plug down at 10:00am. Operator can break off at 4:00pm.
