0305669751 2013-08-20 Final Restoration No Violations My inspection found the well to be plugged and all the production equipment has been removed. The area has been seeded, graded, and vegetation has been established. I met with the landowner and he informed me that he was satisfied with the location and that the lease road and the flow lines can remain. The previous compliance notice can be closed.
0676339680 2011-09-09 Final Restoration No Violations Operator placed stone at the north end of the lease road. Land owner waiver needs to be submitted for the lease road. Operator graded out stone pad that was used by the pump jack which does not allow for use of the field by the land owner. Stone needs removed from site before Final Restoration can be approved.

Operator has failed to restore land affected by oil and gas well activity in the time required by ORC 1509. Stone lease road remains on location. Tank battery stone pad remains on location. Stone pad for the pump jack remains on location. Debris from conductor and surface casing remains on location. Partial roll of liner is on location. 1207563384 2009-10-21 Production Wells No Violations Well ID sign is up. Spacing has not been corrected. On site with Jay Cheslock. Contacted company and informed them that spacing needs to be correct on any future wells. Lease road was stable. There was a thread leak on one of the lines to the separator. Contacted company and requested the leak to be fixed. Well is shut in at this time.
1227292695 2008-11-12 Production Wells Violations Noted On site with Hambden zonining inspector. Well is a pump jack well, tank battery has a mechanical separator, 1 100BBL tank and a 50BBL tank. Separator is 5 feet 8 inches from the production tank, separator needs moved to meet spacing limits. Well doesnt have an ID sign on it. Dike is to low to prevent escape of brine, if a leak occurs. Site has been graded and seeded. Called Tim Levengood regarding work that needs completed. Separator needs to be 10 feet distance from the production tank. ID sign needs posted on tank battery with correct information. Dike needs built up to be sufficient to prevent the escape of brine.

1283262026 2010-08-31 Final Restoration No Violations Well has been plugged. Tank battery is in place. Lease road has not been removed. Rock pad for the pump jack is in place. Called Tim from Industry Standard, waiver will need to be submitted if lease road is to remain. Suggested take the over size rock from the pad to another location on Wilson Mills road and use it to line drainage channel to the road.
