Land owner at 7950 has a DMRM gas meter and a meter supplied to them by the company. |

At this time the operator doesnt know if their well is causing the gas to be in the water wells, but is being proactive regarding the gas in the water wells. At the time I arrived at the well site, operator was pulling tubing from the well. Operator is planning on setting a bridge plug, perforating the 4.5 inch production pipe and cementing the annular space of the well. Operator set bridge plug at 3657 pressure tested plug at 2000 psi for 3 minutes, plug held. Operator perforated the 4.5 inch production casing at 3600-3602 with nine hollow steel carriers. Operator broke circulation through the perforations with salt water. Hole was gassing and producing a light brown oil. Operator then ran sweep through the perforated casing and annular space. Operator then ran 800 sacks of 50/50 pozmix cement. Operator lost circulation during cement job for approximately 5 minutes. Circulation was regained, no cement returned to surface. Majority of the gas was stopped after cementing was complete. Operator could hold back gas at the discharge with his hand. Operator plumbed the annulus into a frac tank on site. Operator will have an employee on site to monitor the well. |

Cheslock requesting my assistance in a natural gas leak and explosion in Bainbridge Township, Geauga County. Insp Lowder was reportedly in route and messages had been left for Insp. Hill. Upon arrival I met with Insp. Lowder, the Bainbridge Fire Chief and Deputy Fire Chief. We discussed canvasing the neighborhood with three man teams including Fire Department and Dominion East Ohio personnel to meter LEL levels in the homes. Several reports indicated that some water wells are located in the basements, which would appear to pose a higher risk. We were also advised that there was a problem at the water well at the Bainbridge Police Department. The Fire Chief stated that he would provide MRM with the inspection results. Insp. Claus was contacted and provided support by researching water well records on the ODNR web site. Insp Hill and I then met with Cal Marks (Transcontinental Oil & Gas) and John Fleming (Range Resources) and inspected their respective wells also located in the general area. By early afternoon the neighborhood canvas results indicated that 12 homes (five on English, five on Scotland and two on Kingwood) had detected levels (above 1% LEL) in either the home or at the water well. These people were advised that Ohio Valley had made hotel reservations for all affected people and that they would be updated as information became available. For the remainder of the day and night, Insp. Hill and I worked with and witnessed the corrective actions that Ohio Valley was undertaking to correct the problem and stem the gas flow. |

Appalachian Well Survey was running a temperature log at the time of my arrival. Quick review of Temperature logs by Tom Wood, showed areas of potential sources of gas at 1684, 1580, 1410, 1310 and 1230. Appalachian Well Survey didnt set a 2nd bridge plug. Operator had ran 30 feet of cement on top of the wiper plug during first squeeze job of the well. Appalachian Well Survey perforated with nine hollow steel carriers from 2628 to 2630 . Universal Well Services broke circulation and then squeezed 800 sacks of 50/50 pozmix cement. During the process of perforating the 4.5 inch pipe and gaining circulation, a hand stated, that three more water wells had blown out. I left the site to check the annulus pressure on permit 21946. Tom Wood felt that there had to be another source, contributing to the gas in the area. Annulus space on permit 21946 had a bull plug and a needle gauge on it. Needle gauge was reading zero. Returned to well site, Universal Well Services was pumping cement. 41 barrel return of cement to surface. After the completion of the cement job, Operator will continue to allow well to vent into frac tank. I spoke to Tom Wood, regarding the well and any issues that they had. Tom indicated that during the cementing of the long string, they lost 700 of cement to the Packer Shell. This was indicated on the bond log. During the fracturing of the Clinton, 44,000 barrels of water and 300 sacks of sand into the job there was communication between the 4.5 inch pipe the annulus. |

Ted and I also attached a weight to the end of the tap, to allow it to pass through the water in the annulus. Tape went down approximately 13 feet without tagging cement. Ted will remove the final retaining ring 03/06/2008. He will also bring 1/4 steel rods to attempt to tag cement and obtain a sample of the cement. Gas bubbling vigorously in the annulus space was heard by myself and Ted. |

Attempted to take a core sample of the cement with steel tube, 1/2 inch of cement in the tube was all that was obtained. Annulus space has approximately 4.5 of water in it. Sample of the water was obtained from the annulus space. Water had 969 ppm Chloride. Gas was still bubbling in the annulus space. Ted reassembled the well head and put retaining ring back on. Ted allow fittings on the annulus space to be lose, to allow gas to escape. 17990 English took static water reading. Static water level was 89 feet. Static water level on 03/05/08 was 88 feet. OVE is pumping well at 20 gpm. LEL reading on the well was 100%. OVE has been pumping the well at 17990 English to allow gas to vent out of the aquifer. |

At the time of this inspection, this well was equipped with a plunger lift assembly and the valves on the wellhead were open. The 8 and 5/8 inch annulus was plumbed above grade, equipped with a pop-off valve and open. The pressure gauge on the annulus valve was reading zero. No issues were observed. |

Pressure test 4.5 inch casing. 17:15 BOP closed pump failure. 18:00 BOP closed at 1200psi. 18:04 4.5 inch casing psi 550psi, 18:06 psi 520psi, 18:10 500psi pressure bleeding at the packing. Test failed. Change over and valve placed on tubing. 18:38 BOP closed at 1450psi. 18:43 4.5 inch casing pressured to 550psi. After 20minute test zero pressure loss. |
