0159524104 | 2008-08-13 | Preliminary Restoration | No Violations | On site with adjacent land owner and Jay Cheslock DMRM supervisor. Operator has graded and seeded site. Operator has used seed fabric in the drainage ditches to hold vegetation. Vegetation is established at this time. Operator encountered a drain tile on the east side of the site. Operator repaired drain tile. Tile discharges into sedimentation pond, land owner was concerned that the water was of poor quality, water has iron in it. Called Shannon at Geauga soil and water to advice that there maybe a storm water run off issue in the area. Adjacent landowner is part of the unit, his property is non-drill. During the drilling process operator cleared trees and brush from the landowners property, a secondary lease agreement was made between the landowner and company. Landowner wants trees replaced, this is a civil issues between the company and the landowner. Operator has complete drainage control repairs on the adjacent landowners property, work to be completed in lease agreement. Landowner stated that he has had a change in the quality of water coming from his well. Landowner stated that he had an agreement in the lease if he has a change in well water quality. Operator had well tested before drilling, I advised landowner to contact the company for a post test of his well water, if the company does not correct the issues to contact DMRM and we will start an investigation. |
0342659122 | 2008-05-09 | Drill / Deepen / Reopen | No Violations | On site to meet with adjacent landowner and company. Mr. Roller is the landowner to the south of the site. His property is in the drilling unit. He is a no drill lease holder. Operator has cleared an area approximately 30 X 50 on his property. Operator is planning on constructing part of the pit on his property. Land owner is first upset that they have cleared part of his land without permission, second that drainage flowing towards his property has been changed. Per the landowner any changes to drainage in the area increase flooding on his property. There is a sediment pond on the Petronzio property adjacent to the site. It was agreed that the pond would be cleaned out and a spillway constructed for the pond once drilling is complete. Ponds spill way has been washed out. A well constructed spillway will increase the capacity of the pond by about a foot in depth. Operator agrees to construct an additional catch basin on Mr. Rollers property to slow water flowing towards him. Mr. Roller has a small pond on his property that all the drainage form the affected area flows into. It was agreed between the operator and the land owner, that the entrance points to the pond will be improved and rock lined. Landowner was concerned about what types of trees were removed from his property and the quality of the trees. I was on site when property was being cleared and for the pre-site. Land was covered with brush: multiflora rose, honeysuckle and small trees, many of which had multiple stems. All cleared brush and trees were of a size that could be chipped. Operator talked over reclamation with Mr. Roller, regarding planting hardwoods on his property to make up for the clearing. Operator will submit a new lease agreement to Mr. Roller for his review. Mr. Roller agrees that part of the pit can be constructed on his property. |
0355280838 | 2008-05-12 | Drill / Deepen / Reopen | No Violations | On site at 10:00pm for CND job. Operator ran two joints of pipe, third joint was getting hung up. Operator attempted to turn joint down. Wash joint down, joint was hitting solid. Operator decided to trip pipe out and clean hole with bit. Operator will call when hole is cleaned. Left site. |
0515085684 | 2010-06-03 | Production Wells | No Violations | On site with Geauga Soil and Water to review drainage controls. Mr. Roller, a landowner to the west of the location had flooding over the weekend. Mary Roller contacted me 06/01/2010 regarding the flooding. National Weather Charts showed that the flood event was a 250 year event. During the reclamation of the site the operator built two drainage swales on the affected area, directing drainage north and west. One swale discharges directly into a pond on Mr. Rollers property. The 2nd swale discharges into a pond, north west of the well site. The pond also discharges into the pond on Mr. Rollers property. The pond adjacent to the well is approximately 35 feet X 31 feet (0.02 acres,) with an unknown depth, the pond does not have a primary or emergency spillway. This pond also receives drainage from the north, off of the shopping centers parking lot. Well site receives drainage from a parking lot to the east of the location, that flows west across it. The pond on Mr. Rollers property is approximately 40 feet X 30 feet and 6 feet deep (0.027 acres.) The pond on Mr. Rollers property has a 4 inch over flow pipe in it, no primary or emergency spillway. The pipe flows into a drain tile that flows to the road. Well location: vegetation is established on location. Vegetation is established in the drainage swales. Grass in swales had been laid down from the flow of water. Drainage for the surrounding area flows north across the oil and gas well affected area and west. Landowners south of Mr. Rollers property have a drainage swale cut north/south to divert drainage north onto Mr. Rollers property. During well reclamation Mr. Roller had Cutter Exploration cut a drainage ditch approximately 6 to 8 inches deep and 6 to 12 inches wide intending to connect the adjacent landowners drainage swale with his pond. This ditch is intended to prevent water from flowing between the homes and developing its own drainage swale. The drainage swale has two bends in it. Mr. Roller |
0690520733 | 2010-06-01 | Production Wells | No Violations | On site with Chester Zoning to review drainage controls. Mr. Roller, landowner to the west of the location had flooding over the weekend. Mary Roller contacted me regarding the flooding. The flood event was between a 250 and 500 year event. Operator during the reclamation of the site built two drainage swales on the affected area, directing drainage north and west. One swale discharges directly into the pond on Mr. Rollers property, the adjacent land owner. The 2nd swale discharges into the pond, north west of the well site. The pond also discharges into the pond on Mr. Rollers property. The pond adjacent to the well is approximately 35 feet X 31 feet unknown depth, no primary or emergency spillway. This pond also receives drainage from the north, off of the shopping centers parking lot. Site receives drainage from parking lot to the east of the location. The pond on Mr. Rollers property is approximately 40 feet X 30 feet and 6 feet deep. The pond on Mr. Rollers property has a 4 inch over flow pipe in it, no primary or emergency spillway. The pipe flows into a drain tile that flows to the road. Well location: vegetation is established on location. Vegetation is established in the drainage swales. Grass in swales had been laid down from the flow of water. Note: the oil and gas well site doesnt contribute additional drainage to the area, the velocity of the water may have changed coming off of the affected area. Drainage for the surrounding area flows north across oil and gas well affected area and west. Landowners south of Mr. Rollers property have a drainage swale cut north/south to divert drainage north onto Mr. Rollers property. During well reclamation Mr. Roller had Cutter Exploration cut a drainage ditch approximately 6 to 8 inches deep and 6 to 12 inches wide attempting to connect the adjacent landowners drainage swale with his pond. 1193009415 | 2008-05-12 | Drill / Deepen / Reopen | No Violations | On site with landowner, Mr. Roller, adjacent landowner who is in the unit. His lease is a non-drill lease. Operator has built first pit. Pit had surface run off in the bottom, didnt appear any ground water was entering the pit. Landowner was upset additional topsoil had been stockpiled on his property past the agreed amount. Landowner was concerned how much of the pit was going to be on his property. Asked landowner if he want them to stop. Landowner felt that the damage had been done, just wants them to get done. At this time safety fence is not up. Additional drainage controls are need. |
1210784541 | 2008-05-14 | Drill / Deepen / Reopen | No Violations | On site for surface job. Operator has finished running pipe before I arrived, they had completed drilling earlier than planned. Surface pipe was in the hole, operator was circulating on it. Good circulation. One section of 8.625 pipe was on the rack, the pipe was wielded in the center. Spoke to Roy Poulson and a hand on the rig, there had been more pipe like that on the rack and pipe had been ran. Spoke to fellow inspector and supervisor, it is policy that wielded pipe not be ran. Supervisor suggested that if they have evidence that the pipe has been pressure tested and is sound that they can leave it, this time. Pipe must be as good if not better than standard pipe, that is not wielded. Spoke to Lynn Strazi consultant for Cutter, he did not have certification regarding the pipe at this time. Spoke with Dan Allen from Miller Supply. Pipe was gas line pipe. Pipe was cut in 40 sections. New thread collars were wielded to the cut ends of the pipe, other ends were threaded. The wield on the pipe, is where the pipe was wielded when the pipe was installed as a gas line. I asked him if the pipe had been pressure tested, no it had not been. Pipe was new R-3, 20lb pipe when it was installed as a pipe line. Asked Dan if he knew how long it had been in the ground, no he didnt know. I asked Dan if he knew at what pressure it had been operated at, he did not know. I asked Dan Allen if he knew the condition of the pipe right now as it is in the hole, no he did not know. I order pipe to be pulled from the hole and new pipe brought on site. First joint of pipe pulled was a wielded section of pipe. Pipe was pitted and had rust scale. Majority of the joints pulled were wielded pipe, 5, 7 and 8 sections of pipe were wielded pipe. Those joints were pitted and had rust scale. |
1211031790 | 2008-05-17 | Drill / Deepen / Reopen | No Violations | Drill depth 2458. Pits are in good repair. Operator is drilling on fluid. BOP is in place. Pit fluid level is good 2 plus feet of free board. Eric Poulson on site. Drainage controls are in place and functional. |
1219780200 | 2008-08-25 | Drill / Deepen / Reopen | No Violations | On site with Shannon Mullins from Soil and Water, Dan from Cutter, Jay Cheslock DMRM supervisor. On site regarding reclamation on site and a drain tile that was encountered during construction of the site. Drain tile discharges into a settling pond on the west side of the site, bio-film is forming on the surface of the water. There is iron precipitating out of the water and staining the stone in the drainage channel. Tested for Cl- , Cl- reading of 1.8 (52ppm) on low range quantab, ph of the water was 8. Dan, is going to add limestone to the drainage channel, to attempt to clean up the iron. Site will be reassessed in the spring, the 2nd option will be to disconnect the drain tile where it comes out of the parking lot hill side and allow it to flow over the ground to the pond. The hope is that the ground will act like a filter. I called Soil and water regarding this issue, the water is coming from offsite and flows across the site, possible storm water issue. The two adjacent neighbors to the west have complained to the company regarding a change in their well water. Cutter is addressing the issue at this time. Spoke to Dan on 08/26/08 referred Dan to Ahmed Hawari in regards to the water wells. First landowner is complaining of iron film in his well, the 2nd owner is complaining of sand in their well. |
