The last reported production for this well was submitted in 2015. The well was fenced with a locked man door. The tank battery consisted of one 100 BBL production tank, one 50 BBL poly brine tank and a mechanical separator. The valves on the separator were open and the chart showed production. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter, then plumbed up and topped with a spark arrestor. The tank valves were equipped with flat plugs. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the tank battery and the lease access was stable. No violations were observed. |

The 3 inch flow line at the bottom of 100 BBL tank had been pulled away from the tank causing the leak. Brine hauler was on site 04/21/2010. Leak was discovered by Mark Terpact well tender in the afternoon of 04/21/2010 . Crude oil escaped the tank battery. The crude oil enter a drainage ditch and flowed west for 102 feet. Straw bails and peat moss have been applied in the ditch, to prevent further spread of oil. Vegetation in the ditch is still green, no brine scalding noted at this time. Air readings for the area Zero LEL Zero H2S, taken with PhD*lite. Scott Miller from Scott Miller Contracting, on site for clean up. Scott and I agreed, tank battery will be stripped of equipment, contaminated rock and subsoils will be removed, soils will be removed from the ditch and removed from site. All removed material will be taken to an appropriate disposal facility. Clean clay will be brought onto site and placed in the bottom of the battery, clay will be compacted. Scott is going to trench around the battery and use clay below and above grade to build the dike. While excavating the rock out of the tank battery, it was noted that the north end of the battery was built on top of size 3 and 4 rock with 57 rock on top. The dike was placed on top of the rock. The rock allowed the crude oil to escape the dike. Rock was removed from the battery. Contaminated material excavated during clean up was placed into a roll off dumpsters, that was lined with a plastic liner. |

There was 3 inch nipple at the bottom of the tank that connects into a ball valve, then there was another section of pipe approximately 12 inches long that connects into a 90 degree elbow. From the 90 degree elbow the flow line ran to the north end of the tank battery. The valve was pulled apart at the threads. The meeting was to discuss if the brine hauler pulled away from the battery with the load line still connect to the truck. When leak was located by Mark, per Mark the lease road gate was locked and the man door was locked. The hauler ticket was located under a rock not the usually location of on the tank ladder. Haul tickets and gauges were compared, there was a discrepancy of 3 BBLs of fluid that would suggest the final reading was an estimate. Ergon agreed to pay for oil spill clean up. |

North end of the blow pit, the bottom of the pit had water leaking into it. First 4 feet of the pit was topsoil and clay, the last three feet is a mix of shale and sandstone that is thinly laminated. Were the sandstone and shale met, ground water was entering the pit. Operator is going to back fill 1.5 feet of the pit with clay, to prevent ground water form entering the pit. Pit will need to be betonite coated, padded with straw before lined. Cuttings from the blow pit may need to be removed from site, once drilling is completed. |

The tank battery consists of a 100 barrel steel production tank, a 50 barrel poly brine tank, a dominion gas sales line, and a mechanical separator. The separator had a 16 day production chart in the meter box. All of the valves had locks on them. The drip lines of the sales line were all plumbed back into the 100 barrel tank. The production tank was vented to a 55 gallon drum charcoal filter, due to the area being zoned urban. The proper identification sign was on both the well head and the tank battery. The lease road had a locked gate on it and was in good condition. |
