0000345620 2017-02-06 UrbanProduction Wells No Violations At the time of this inspection, the well was observed to be equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced and locked. The last reported production for this well was submitted in 2015. The tank battery consisted of one 100 BBL production tank, one 50 BBL steel brine tank and a mechanical separator. The valves on the separator were open and the chart showed production. The separator union valves were seeping some oil and needed to be tightened up. The tank valves were equipped with flat plugs. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter then plumbed up and topped with a spark arrestor. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the tank battery. The lease access was stable. I called and spoke with Mark Turpak of Cutter about the leaking hammer unions and getting those tightened.

The the well was observed to be equipped with a plunger lift assembly and the valves on the wellhead were open. The well was fenced and locked. The separator union valve was no longer seeping and has been repaired. The tank valves were equipped with flat plugs. The proper, legible signage was located at the tank battery. The lease access was stable. No violations were observed.
0886098202 2013-03-07 Production Wells No Violations I conducted an inspection pursuant to a call from a landowner indicating a suspected release of oil from the unit into Dominion/East Ohio (Dominion) gas transmission lines. Reportedly, several homes located near the intersection of Cedar Road and County Line Road had gas service interruption due to oil accumulation in the line, at the meters. Prior to arrival, I was contacted by Gates Mills Fire Chief Tom Robinson, and I indicated to him that ODNR was aware of the report, and I was responding to the site. Also, prior to my arrival, I contacted Mike with Cutter Exploration (Cutter) and advised him of the report, and he indicated that he would in turn contact his service person. Upon arrival I encountered service crews from Dominion working to restore service at the affected homes. They indicated that they had shut off the service connection at the metering run. After my arrival Mark from Cutter arrived to evaluate the situation. He indicated that the producing unit had been returned to service within the last week, and only began producing fluid yesterday (3/6). He thinks that there may have been residual oil in the line between the mechanical separator and the metering run which got kicked into the service line. Based on my discussions with Mike and Mark, they will keep the unit offline until they add one or more additional pressure shut offs. The Dominion crews were able to re-establish service at the five affected homes at around 5:30 PM. I told Mike that we would follow up and confirm that the problem was corrected prior to returning the unit to service.

The tank battery was comprised of one 100 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The tank valves were equipped with plugs and the vent stack on the 100 BBL tank was plumbed into a 55 gallon charcoal filter, then plumbed up and topped with a spark arrestor. The diked tank battery was fenced with a locked man door. No leaks were observed at the tank battery location. Legible identification was present at the tank battery and the lease access was stable.
