The annulus was open to a pop off valve and was plumbed into the vent line for a charcoal filter. The tank battery was surrounded by a locked wooden fence and a proper dike. The tank battery consists of four 100 barrel production tanks, a 50 barrel poly tank, and two mechanical separators. The vent pipes on the productions tanks were run to two 55 gallon barrel charcoal filters. The tank battery is being shared with permit number 2-2123. The proper identification sign is on the tank battery for permit number 2-2088, but there was no sign on location for the tanks and separator for permit number 2-2123. The lease road is in good condition. |

During logging, logging tool was differentially hung in hole, when attempting to retrieve tool, cable partied. Lost logging toll down hole Monday 04/20/2009. Operator has made three trips to retrieve tool. Tool parted, retrieved density tool, source and source pad still in hole. Operator advised to contact NRC. Spoke with Sattar Lodhi from NRC, Ohio is an agreement state, contact the Ohio Department of Health. Ohio Department of Health has Jurisdiction, spoke with Steve James ODH advised Steve we are planning on running in with a magnet to retrieve remainder of tool. Steve is going to contact supervisor will call back, waited for return call before running in for fourth trip. Operator has made three attempts to retrieve tool. It was agreed running in for a fourth trip would risk damaging the remainder of the tool. Superior has been monitoring the hole and circulation for radiation nothing but back ground has been detected. It was agreed to cement source and source pad in place rather than continue to attempt to retrieve it and possible break source open. These are notes given to me by Norburt Lowder: Tom Hill left site I covered the 4.5 cement job. 4.5 set at- Well TD 3825 with logging tool source at TD 3824 Procedure dictated by Ohio Dept of Health, Mike Snee. Todd Mobley on site consultant for Superior Well Service as there RAD safety person. Procedure as follows use 7 7/8 bit wielded to stub of 4.5 approx 12 collar then latch down plug, then 4.5 casing. 1 run to bottom 3815 to set 51 of cement. Mixed and pumped 50 Sk of cement pulled up to 3765 washed 4.5 then cement to 2678 approx 210 sks 58 BBl displacement, 700 fill up. Log to 3824 hot. Lockport 3142 log. Good circulation. Pipe was 4.5, 11.60lb new. Should be noted 50 sack of Class As yield is 1.18cuft: 50sacksX 1.18= 59cuft. 7 and 7/8 hole is .3382 per ln ft. 59cuft/.3382= 174 of fill up. |

The tank battery consisted of four 100 BBL tanks, one 50 BBL poly brine tank, and two mechanical separators. The valves on the separators were open and the charts showed production. There were proper lids and hatches on the production tanks. Spark arrestors were installed on the productions tanks and a charcoal filter was plumbed into the vent stack. The tank battery was diked and fenced with a locked man door. The tank battery is shared with well 2-2123. Signage needs to be updated at the tank battery to reflect information on both wells and signage needs added to each well location. The lease access was stable. I contacted Mark Turpak of Cutter Exploration about updating and installing signage at the well and tank battery locations. |
