0626814283 | 2009-12-22 | Drill / Deepen / Reopen | No Violations | On site to follow up on the status of the pits. Operator is waiting on a 3rd load of fines. Pits are not sufficiently solidified. 3rd load will improve the conditions. Load will be on site between 12 and 1pm. |
0681671841 | 2009-12-09 | Drill / Deepen / Reopen | No Violations | Site has drainage issues, site is in a hallow and has a large water shed. The landowner is constructing a pond 75 to 100 from the well location. Landowner has affected approximately an acre with the construction of the pond. Pond construction has not been completed, area is barren and has exposed sand, gravel and clay. There is a drainage swale that runs under the roadway to the site, a drainage swale that flows south east cutting across the site, the operator had to divert drainage swale around drill site. Drainage was flowing across the lease road into the pond. Pond has a stream that flows into it form the east. Operator constructed a diversion ditch on the back side of pits, to divert water away from pits. Area has received rain and snow the past few days, stream and drainage swale flow was vigorous. Majority of the drainage from around the site is directed into the pond. The pond discharges into the river to the south of the location. Operator has constructed drainage controls in the pond outlet, installed silt fence around the site and has used rock and wood chips to control offsite sedimentation. Operator is only responsible for drainage for the area affected by the drilling rig, but has attempted to control all site drainage. Drainage discharging into the river was running clear. Pits were under construction, clay, sand and gravel was noted, no ground water noted. Operator filled pits with water from the pond and stream. Operator spudded CND, drilling on fresh water. Glacial till was encountered. Left site operator will contact me when they are going to cement conductor. Permit was posted in the dog house. Lease road was stable. Safety signs were up. hand railings on rig were up. Pit liners were installed correctly. |
0724493768 | 2009-12-29 | Administrative Inspection | No Violations | E-mail from Warren Shoenfelt Denzine #1, 12/18 Shut in Annulus at 12pm w/ 50# relief. 12/22 0 psi, 12/26 22psi, 12/29 42 psi. Head packing was leaking blew down, tighten top ring, change 50# relief to 75# relief and shut in at 1pm. Max allowable pressure = 80psi. |
0771350012 | 2009-12-21 | Drill / Deepen / Reopen | No Violations | On site for pit solidification. Employee from Hugh Palmer Excavating on site. Two loads of fines have been mixed into pits. South end of work pit was very thin. Equipment operator was in the process of mixing fines into blow pit. Will check in the morning to see if pits thicken up. Operator had made a pad in the center of the work pit and had excavator parked on top of it. That section of the pit and north end of the work pit was sufficiently solidified. Lease road was stable. Drainage controls were in place. Annulus pressure was zero. Pop-off and pressure gauge was on the annulus. |
1096139821 | 2017-02-28 | UrbanProduction Wells | No Violations | At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were open. There was a domestic line with a meter and a regulator plumbed off of the 4.5 inch casing with open valves. The well was fenced with a locked man door. The last reported production for this well was in 2015. The tank battery consisted of one 100 BBL production tank, one 50 BBL poly brine tank, a mechanical separator and a sales line. The valves on the separator were open and the chart showed production. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter and then topped with a spark arrestor. Tank valves were equipped with flat plugs and locked. The tank battery was diked and fenced with a locked man door. Proper, legible signage was located at the well and tank battery. Lease access was stable. No violations observed. |
1152699912 | 2010-10-27 | Preliminary Restoration | No Violations | Chain link fencing around the wellhead, board on board around the tank battery. Area around the well has been graded. Spacing is correct. Operator plumbed a 2 inch line from the top of the 100Bbl tank into the top of the poly tank. The line circumvents the charcoal filters and pressure relief valves. Line needs removed or check valve installed. |
1221552082 | 2009-12-17 | Drill / Deepen / Reopen | No Violations | On site for long string cement. Operator conditioned hole, broke circulation, ran cement, displaced and locked plug down. No issues, good circulation. No back feed, plug locked down tight. Observed cement during mixing, cement had a good texture to it while mixing.. Pits are in good repair. Drainage controls are up. Rig is rigging down for move. Pipe was new 10.5 lb pipe. Operator was draining fresh water tank into drainage ditch. Tested water for CL-, 35ppm, water was from the river. |
1260974880 | 2009-12-14 | Drill / Deepen / Reopen | No Violations | Mr. Baker is the driller this day. Operator is drilling in the lime at this time. BOP is in place, control line to the BOP was not hooked up, questioned Mr. Baker about the line, he stated he tested the BOP to 400psi repaired any leaks. They keep the line disconnected to prevent damage to it. Line has a quick connect fitting on it, they would hook it up in seconds if there was an issue. Choke line was not installed, operator is drilling on fluid. Pits were in good repair. Operator had pulled up surface cuttings and place them on an apron made from the pit liner. Pits are small and operator had to make room for more cuttings. Checked Fire extinguishers in the dog house, there were two, both were in the green. One extinguisher was on the boom, could not inspect. Safety fence was up. Lease road was stable. Drainage controls were in place. Permit was posted. Safety signs were posted. Site was in good repair. |
1291396863 | 2010-12-03 | UrbanProduction Wells | No Violations | Annulus pressure is 44psi, operator is producing domestic gas off of the backside, Domestic meter is on the annulus gas. Fence is up around the well. Drainage (regarding wetland damage and possible illegal mining by the land owner, starting at the east side of the permit; drainage is flowing from the affected wetland east of the well. The drainage flows into the impoundment area then flows south/ south east to the stream. Drainage was running clear. Drainage channel is starting to form. A second drainage flow was also flowing out of the impoundment area, then flows slightly south/ southwest. This drainage channel after it discharges out of the culvert is well defined and was flowing clear. Iron staining was noted in the small pool of water at the culvert discharge. North west of the well, there is a hallow, drainage was flowing east across the lease road and south along what would have been the west edge of the pits. Drainage was running clear. Note: Impoundment is not complete and there was no standing water, water was flowing threw the affected area. Site had an inch of snow on it. Lease road was stable. Tank battery, operator has taken a bull plug out of the top of the 100Bbl tank and hard plumbed the tank into the poly tank. Operator has also plumbed a pop off onto the charcoal filter. Plumbing from the steel tank to the poly tank needs a check valve. Valve on the transfer line from the bottom of the poly to the top of the 100Bbl was open. |
1310582773 | 2011-07-13 | Preliminary Restoration | No Violations | Well head is fenced and man door is locked. Lease road has a gate. Tank battery is fenced and man door is locked.
