Clay will be mixed into the blow pit before lining. Fresh water source is the stream on RT 168. West side of the site slopes to an intermittent stream, that flows north adjacent to the site. West side of the site is 3 feet of feel material the edge is loss fill. Contacted operator advised that drain controls need installed. Spoke to excavator operator, Company Wad Pol,) advised operator drain controls need installed along the west side of the site to prevent off site sedimentation. |

Drilling mud was 2 feet below surface, operator broke circulation, ran 200lbs of gel and one bag of cotton seed hauls ahead of the cement. Cement water pH was 7. Operator ran cement then switch over to displace. Cement returned to surface as soon as displacement started. 18 Bbl of good cement returned to surface. Once plug was down and operator shut down cement in the annulus fell back. I had the rig hands shovel remaining cement in the ditch into the annulus space. Requested that the operator tag cement and report depth of fall back. Corrective actions will need to be taken to fill annulus with cement. Total loss of fluid unknown at least 6 truck loads. |

Did not have operator grout from the top after the long string. Operator will need to mix and grout at a slow rate to repair the surface cement. Operator conditioned hole for 45 minutes before cementing. Operator had conditioned the hole before tripping out the drill string also. Good circulation during conditioning. From the log; Sharon Sandstone top 15 bottom 120, Berea top 270 bottom 334 Lockport top 3486. Long string pipe was 11.6 lb R-3 used pipe. Operator broke circulation, mixed cement and ran cement down the long string. During cementing, a gas kick was taken at surface. Kick was circulated out during displacement. Hole was static after displacement. No breaks of circulation during cementing. Operator plugged rat hole with 20 sacks of Class A cement. Cement held at surface. |

The dike is in good repair. The lease road gate was open. The identification sign needs filled out. There are two holes, of unknown depth, along the last quarter section of the lease road. Water is flowing into these holes, which could possible be broken field tiles. I contacted the well owner in regards to the identification sign and possible tile repairs. |
