0003448970 | 2012-05-13 | Urban Drill / Deepen / Reopen | No Violations | I was on site at 15:30 for Blow Out Preventer equipment test. Test was underway when I arrived on the rig floor. Robert Roush, Inspector 1 had arrived before me and rig started test. Paulson Drilling closed the blow out preventer at 500psi and charged the annulus to 175psi. First test failed, bull plug at the bottom of the BOP was leaking. Test was shut down to tighten fittings. Second test, annulus charged to 175 psi. I had to remind the driller to shut valve to the drill string line. I didnt want the air compressors holding pressure, system needed boxed in. Blow out preventer passed 2nd test. BOP had lost 5% of pressure, the bull plug still had a minor leak. Bull plug was covering threads that the choke screws into. Nitrogen bottle for the Blow Out Preventer was fully charged. Test performed at lower pressures do to shallow surface casing and the potential for damaging the Blow Out Preventer bladder from string lift. Pits and pit liners where in good repair. There was sufficient free board in the pits, greater than 2 feet. Safety fence was up around the pits. Lease road was stable. |

The well is fenced but needs a new lock on the man door. The well also needs signage. The tank battery consisted of four, 100 BBL production tanks, two, 50 BBL poly brine tanks, and two mechanical separators. Production tanks had lightning protection and charcoal filters. The tank battery is diked and fenced. The tank battery is shared with well 22112. There is signage at the entrance to the wellsite, however signage should also be added at the well and tank battery. The lease access is stable. I called Mark Torpak of Cutter with regards to installation of additional signage at the well and tank battery. |

The production tanks a vented through 55 gallon charcoal filters and then un into spark arrestors. The proper identification is located on the wellhead and the tank battery. The lease road is in stable condition and is being used as a residential driveway. The site has been graded, seeded, and vegetation has been established. Preliminary restoration is passed. |

Review of the bond log showed the top of the cement at 3,060 feet. The bond long showed good bond over and above the producing formation. Bottom hole temperature was 102.5F. The top perforation is 3,650 feet and the bottom perforation is 3,718 feet. The top of the Lockport is 3,132. The annulus was static before the job was started. Highest annulus pressure reading during the 5 day test was, 12psi. Job went as planned other than needed schedule adjustments due to the lack of water available for the job. Operator had to increase slurry mix earlier in the job, to assure there was enough water for flush. Annulus was static during and after the fracturing. |

Operator failed to notify me that they were solidifying the pits on 05/22/12. I had advised fellow inspector Mike Dinsmore to performing a follow up inspection on the site 05/22/12, to check the status of the pits and pit liners. Mike had informed me that the operator was in the process of solidifying the pits. Operators consultant, Bob Randall was notified, by me, that I had not received notification of pit solidification. I informed Bob that pits where not to be closed until I could check the status of the solidification on 05/23/12. My inspection found the work pit to be solidified and topsoil redistributed over it. The topsoil was placed on the pit to allow the excavator to track onto the top of the pit, to allow it to work solidification material into the blow pit. Bob had informed me that this had been done. Work pit was able to support the weight of an excavator. Blow pit had been solidified and was open. I check the solidification by throwing a few pieces of number 3 limestone into the pit. East end of the pit, the limestone bounced off the solidified pit material. West side of the pit, the pieces of limestone when thrown, sank less than an inch in the surface. The operator had mixed three loads of fines into the pits to solidify them. The well annulus had 11 psi on it. Operator has a 50psi pop-off in place on the annulus. The annulus is locked open. |

Petroset broke circulation, ran 220 sacks of Class A (2% CaCl) cement. During displacement a gas kick was circulated out. No loss of circulation during cementing. Plug locked down at 1500 psi and held for 3 minutes. Note: The hole took water in the Newburg and gas was encountered at 1925 during drilling. |

Paulson Drilling was completing the running of the 8.625 casing when I arrived on location. There had been 8 joints of 8.625 casing on the pipe racks during the conductor cementing. Operator was running 7 joints of 8.625 casing and there was one joint left on the rack when task was completed. Paulson Drilling attached the cement head to the casing, then connected to the head and broke circulation to load and condition the hole. Petroset Cementing on location to cement the surface casing. Bob Randal the consultant for Cutter Exploration decided to mix cement until it returned to surface, then displaced. Cementing operations: cement water pH was 6. Operator broke circulation then ran one bag of cotton seed hauls ahead of the cement. Operator mixed and started to pump Class A Cement with 3% CaCl. Cement weight taken with Division scales during the mixing of cement. The two weights taken during the job, both had a weight of 15.1 lbs. 15.1 lbs per gallon is within the 5% margin of the allowable limit for class A Cement. Scales had been cleaned that day and calibrated with fresh water, at 8.3 lbs per gallon before the job. Cement returned to surface, operator switched over and displaced. 12 barrels of good cement returned to surface. Operator instructed not to break off wellhead until 8 hours had passed. |

Trees and brush had been chipped and stockpiled for the landowner. Topsoil was stockpiled north and south of the location. Operator constructed a diversion that wrapped around the location starting at the north then ran along the west edge of the location to the south end where it discharged into a sump. Operator constructed a similar diversion ditch on the east/ south east end of the location. Sump discharge was controlled with silt fence. Lease road was rock to the location. Mufflers were on all equipment. The rig was oriented east/west. Drilling pits were constructed north of the rig. Drill pits were constructed in tight brown clay. No water noticed entering the pits, pits were dry. Had to remind the operator to slope the pit sides during construction. Pit liners installed with sufficient slack. Fresh water for drilling was from a pond east of the location. Permit was reroute to the location and will be posted before drilling starts on Saturday, per Roy Paulson. Permit identification sign was posted at the entrance of the lease road. |

Mr. Palmer informed me he has silt fence on location and it will be installed before he leaves the site today. I advised Mr. Palmer that the pine trees around the location maybe a safety hazar since the soil around them has been cut on two sides. I reminded Mr. Palmer that they have had pine trees fall on the rig when constructing their locations in a similar fashion. The lease road was stable and constructed of limestone. The lease road up to the land owners home has been topped with #57 limestone. A permit sign was posted at the entrance of the lease road. |

Airhead was in place. Paulson Drilling uses a modified separator at the end of the discharge line, the separator was in place. Discharge line was greater than 60 feet in length. Safety fence was up around the pits. The lease road was stable and in good repair. Operator had absorbent pads in place where diesel fuel had been spilled at the mud pump. Site conditions where dry. Drainage controls were in place. Permit was posted in the dog house. Fire extinguishers where in place on the rig. Pits and pit liners were in good repair. There was sufficient free board in the pits, greater than 2 feet. |
