1347542076 | 2012-09-10 | Drill / Deepen / Reopen | No Violations | I observed the current site activity to consist of grading of the lease road and laying gravel (actually slag imported from Cleveland). Also, a crew is finishing up digging the mud and recirculation pits. They encountered a fairly stiff, gray clay layer about 8 feet down, and the pit sidewalls are fairly stable, and the bottom is dry. The crew asked whether it was acceptable to line the pits, and I indicated that they looked good and lining was acceptable. The topography of the area drops off to the south towards an adjacent creek and man-made lake. The road crew has established erosion controls with silt fencing along the south boundary for some portions of the lease road. Theres no silt fencing on the upper portion of the lease road, and I discussed this with the site owner (Rod), whos also doing the road grading work. He indicated that the surface drainage in that area flows to a swale which forms a more or less natural stormwater runoff retention area just south of the lease road. I told him we could proceed as is, and evaluate as time goes. |
1348151615 | 2012-09-15 | Drill / Deepen / Reopen | No Violations | I witnessed the cementing of the 8 5/8 inch surface casing. An 11 inch hole was drilled to a total depth of 327 feet. The casing, totaling 306 feet in length, was cemented with 165 sacks of Class A cement and 3% calcium chloride. There were an estimated 19 barrels of cement returns.
The cement operation and well site conditions were acceptable. |
1532525670 | 2018-07-25 | Production Wells | No Violations | At the time of this inspection, the well was equipped with a pumping unit. The valve on the tubing and flow line was open. The production records for this well were current. The tank battery consisted of one 210 BBL tank, one 100 BBL tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. |
1601283781 | 2013-03-27 | Production Wells | No Violations | My inspection found the well to be equipped with a pump jack. The annulus valve was closed. The well and tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. |

2012806703 | 2012-10-22 | Preliminary Restoration | No Violations | The well site has been partially graded and the pits are filled-in. Additional grading and seeding will be necessary before restoration is completed. The wellhead is fitted with a standard valve tree and a pump jack, but the well has not been placed into production. The tank battery is under construction, with the dike completed on three sides. The interior elements of the tank battery (e.g., production tank, brine tank, and mechanical separator) are in place but have not been plumbed to the wellhead. The pressure gauge on the 8.625-inch casing is not showing any pressure. Subcontractors are at the location preparing the production piping. Both the cut bank and fill bank at the well pad may need stabilization to prevent erosion owing to the steep grade. The topping fill material at the culverted stream crossing is very soft and may need additional of additional course grade material. Additionally, the cut bank at the tank battery may need stabilization. I will contact Dave Harker. |
