0096560429 2016-07-07 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Scott Scheiner following up on Compliance Notice-1450448227, we met with DA Waldron President Jerry Reeves, Field Supervisor Ted Cartwright and Tim Brown of Timco to discuss a plan for plugging of the well.
0158114595 2016-07-27 Plug / Plug Back No Violations On the day of my inspection I witnessed Huffman-Bowers Inc. (HBI) rigged up on well with pressure reading at 490 pounds per square inches. HBI opened well controlling the release into a 100 barrel tank until pressure was bled down. HBI then pumped 25 barrels of 9 pounds per gallon brine making well static. HBI then bullheaded a 48 sack Class A Berea cement plug from 1254 feet to 970 feet displacing cement with 15.1 barrels of water and then shut the well in. HBI will return tomorrow to tag plug, rip casing and complete plugging of well.

0411784652 2016-07-26 Production Wells No Violations On the day of my inspection I met with David A Waldron Field Supervisor Ted Cartwright to do a controlled release of the pressure on the well. The well initially had 490 pounds per square inches of pressure on it. The well was blown down to 180 pounds per square inches and was shut-in as fluid was present. Thirty minutes later the well had reached 350 pounds per square inches and will remain shut-in until the rig gets there tomorrow to begin plugging operations.

0580217445 2015-12-23 Administrative Inspection No Violations I received a call from Mark Sparr of DA Waldron concerning NOV-1450448227. Mr. Sparr stating that they are preparing to apply for a new plug permit and would like to transfer the well to the landowner David Bartholow. Mr. Bartholow has shown interest in the well and that they are sending him a letter. If he does not respond to the letter they will plug the well in accordance with OAC 1501:9-11-05(B).
1251844652 2009-09-01 Production Wells No Violations Inspection revealed that well is idle at this time, will contact company and discuss future plans for said well, it is equipped with 12 conductor pipe and 8.625 surface casing and 4.5 production casing with frac valve on it ,there is no flow line hooked up to well no domestic hookup either.
1450448227 2015-12-18 Production Wells Violations Noted On the day of my inspection I witnessed the well was equipped with 8.625 inch casing, 4.5 casing, a 4.5 inch frac valve reduced to 2 inches with all valves in the closed position. There were no production lines or equipment present along with no identification and no production reports submitted to the State since 2010. Records indicate the well was plugged back on October 14, 1985 and a plug and abandon permit was issued on January 2, 2013 expiring on January 2, 2015 with no action taken. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1467728613 2016-07-05 Production Wells No Violations On the day of my inspection following up on Compliance Notice-1450448227, I met with DA Waldron Field Supervisor Ted Cartwright and well pressure was checked revealing 460psi on the 4.5 inch casing and 0psi on the 8.625 inch casing. Mr. Cartwright revealed documentation for the well from 1986 where the well was plugged back to 1254 feet. On March 18, 1986 the well was perforated with 34 shots from 1210 feet to 1202 feet and 400 gallons of 15%HF acid was spotted across the perforations. On March 21, 1986 1196 feet of 4.5 inch 11.6#/ft. was set on a Parmco packer at 1196 feet. On March 24, 1986 due to annular pressure the packer was unseated and reset at 1137 feet resolving the annular pressure. Halliburton then fraced the Berea with 28,000 gallons of water, 19,500lbs of 20/40 Ottawa sand with breakdown pressure at 2500psi, average treating pressure of 1400psi and an average rate of 20bpm. Initial shut-in pressure was 1000psi and 5 minute shut-in pressure at 880psi. The well was flowed back, swabbed and monitored but never put online. Mr. Cartwright stated that they are pushing back plugging this week until they can come up with a plan to successfully plug the well without the incident.
1469791880 2016-07-29 Plug / Plug Back No Violations On the day of my inspection I witnessed Huffman-Bowers Inc. (HBI) open the well revealing zero pressure. HBI cut the 8.625 inch wellhead ring off and was unable to free casing which was ripped at 870 feet. HBI unsuccessfully tried to rip casing again at 830 feet and the job was shut down until Monday.
1470051344 2016-08-01 Plug / Plug Back No Violations On the day of my inspection I witnessed Huffman-Bowers Inc. (HBI) open the well revealing zero pressure. Timco Inc. was onsite and tripped inside the 4.5 inch casing and shot the casing at 785 feet. HBI tripped the 4.5 inch casing to 200 feet. HBI gained circulation and pumped a 74 sacks 200 foot Class A surface plug from 200 feet to surface. HBI tripped remaining 4.5 casing tubing, topped off with 10 sacks of Class A cement and cement stayed at the surface.
1543445718 2016-06-24 Production Wells No Violations On the day of my inspection following up on Compliance Notice-1450448227, I witnessed there was no change in the status of the well. I sent Mark Sparr of DA Waldron a request for an update as the completed action for plugging of the well was on 6/17/16. DA Waldron President Jerry Reeves called and discussion was held on plugging of the well. Mr. Reeves has received a bid and has asked for resubmittal of the bid. I stated that they need to get me a timeline for completion of plugging to prevent further action. Mr. Reeves stated that they will get things moving as soon as possible.
1670766810 2016-07-28 Plug / Plug Back No Violations On the day of my inspection I witnessed Huffman-Bowers Inc. (HBI) open the frac valve revealing zero pressure present. HBI tripped inside the 4.5 inch casing, tagged the cement plug at 970 feet and tripped back out. HBI tripped back inside the 4.5 inch casing, gained telegraph and ripped casing at 870 feet. HBI was unable to remove 8.625 inch wellhead ring with lightning and storms moving in the job was shut down until tomorrow.
