A 4 foot diameter area of contamination was present at the wellhead. The production equipment included 37bbl polymer tank, a 2 inch sales meter with .25 inch orifice and a chart recorder all within an earthen dike. 3-55 gallon drums were within the dike with contamination present around the drums and near the load line on the polymer tank. No identification was present and production records were current. Owner/operator shall post identification in accordance with OAC 1501:9-9-05 (A) (10), remove drums, make necessary repairs, remove contamination, dispose of properly and replace with clean fill in accordance with ORC 1501:9-9-02. |
