| 1585632192 | 2013-04-17 | Production Wells | No Violations | On the day of my inspection I reviewed the Divisions production records and found the year of 2012 has not been posted for this well, I made a phone call at this number 614-459-1025 which was posted on the tank, I left a message for them to return my call and update me on the 2012 production records. At the well site there is four and a half inch casing with one and a half inch tubing standing from the ground, the valves are open on the well head, there is production line leading from the well into a separator/ chart and (1) one hundred barrel production tank setting inside a containment dike, the I.D. is posted on the tank. |

I met with the land owner who stated that the well was plugged in 2006 and that his brother owns a well 34059227020000 on Grape Hollow Road in which there has been confusion on in the past between the two wells. |
