0140143347 2018-06-06 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration. During my inspection the production equipment had been removed and the pit had been closed. The location had been final graded. Oil had resurfaced from the closed pit leaving oil saturated soils in 20 feet by 30 feet area. I called Corey McCudden with Northwood Energy and left a detailed voicemail message explaining the situation. I also called and spoke with Mike Corp with Corp Energy a contractor for Northwood and he stated that they would get to the location as soon as possible and remediate the contamination and seed and mulch the location.
0335106835 2018-02-26 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Ryan Armstrong, I witnessed that Buxton Well Service had not arrived yet for plugging operations. A phone call was placed to Derrick Buxton who stated that they were en route to the location and would be rigged up later today.
0672083853 2015-05-28 Production Wells No Violations On the day of my inspection following up on NOV-1431451240, I witnessed the production equipment included a 100bbl tank, a horizontal separator, a logically controlled motor valve, a 2 inch sales meter with a .75 inch orifice and a chart recorder within an earthen dike. The dump line riser has been replaced and the 3 foot by 12 foot area inside the dike near the line has not been remediated. The well was equipped with 8.625 inch casing, 4.5 inch casing 1.5 inch tubing, a plunger lift system and domestic gas service with valves in the open position on the tubing and 1 of 2 valves open on the casing supplying the domestic gas. The 3 foot by 3 foot area of stressed vegetation near the wellhead has not been remediated. Notification was made to Bill Arnholt requesting a time for when work would be completed.

Past inspection found oil saturated soils near the closed pit that was used for plugging. During my inspection today, the contaminated soils have been removed, the surface was regraded, mulched, and seeded. The rest of the location was restored with thick vegetation in place. All debris and equipment have been removed. The location has met the requirements for final restoration.
0948640876 2018-02-28 Plug / Plug Back No Violations On the day of my inspection I witnessed that Buxton Well Service (BWS) was rigged up and tripped out 4.5 inch diameter casing to 4189 feet with no Hydrogen Sulfide (H2S) detected. Formation Cementing pumped a 60 sack 223 Class A Big Lime cement plug with 1% gel, 1% CaCl, weighing 15.2 pound per gallon from 4189 feet to 3966 feet. Circulation was gained and cement was displaced with 63 barrels of water. BWS tripped 4.5 inch diameter casing to 1537 feet. Formation gained circulation and pumped a 55 sack 200 foot Class A Bottom of the surface cement plug with 1% gel, 1% CaCl, weighing 15.2 pound per gallon from 1537 feet to 1337 feet. Cement was displaced with 21.2 barrels of water. BWS tripped 4.5 inch diameter casing out and pumped a 60 sack 192 foot Class A surface cement plug with 1% gel, 1% CaCl, weighing 15.2 pound per gallon from 192 feet to surface circulating cement to surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.

I spoke with Mike Corp with Northwood energy and he stated they would work on the location this week and get it up to final restoration requirements.

1367355847 2013-04-30 Production Wells No Violations On the day of my inspection I reviewed the Division of Oil and Gas production records and found there were no postings for 2012 at this time, I meet with Ron Moore the pumper for Northwood Energy, A landowner called him with a concern of an odor around the well head, the landowner wasnt sure if it was H2S. Ron and I walked up to the well head with our H2S detectors turned on, there was no smell of gas, the detector did not register any H2S, there is 8.625 casing with 4.5 production casing standing from the ground, there is production line leading from the well into a separator/ drip/ and (1) one hundred barrel production tank setting inside a containment dike, there is a domestic gas line and meter coming from the well. The I.D. is posted on the tank.

1431451240 2015-05-12 Production Wells Violations Noted On the day of my inspection I witnessed the production equipment included a 100bbl tank, a horizontal separator, a logically controlled motor valve, a 2 inch sales meter with a .75 inch orifice and a chart recorder within an earthen dike. The dump line riser has been repaired with a band clamp and a 3 foot by 12 foot area inside the dike near the tank and the dump line from the separator was contaminated with oil. The well was equipped with 8.625 inch casing, 4.5 inch casing 1.5 inch tubing, a plunger lift system and domestic gas service with valves in the open position on the tubing and 1 of 2 valves open on the casing supplying the domestic gas. There was 3 foot by 3 foot area of stressed vegetation near the wellhead with traces of oil. Owner/Operator shall remove contamination, dispose of properly, replace dump line riser and replace with clean fill, seed and straw. Identification was present and production records were current.

1519743094 2018-02-27 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Ryan Armstrong, I witnessed that Buxton Well Service (BWS) was rigged up and was tripping out 1.5 inch diameter tubing from the wellbore. I met with Derrick Buxton and he stated that the backside had set his Hydrogen Sulfide (H2S) detector off with a high reading. It was recommended that the well be treated with Sulfa-clear prior to cementing. Sweeney Services was called out and pumped 200 gallons of Sulfa-clear down the 8.625 inch diameter casing. BWS finished tripping out 1.5 inch diameter tubing and Formation Cementing arrived. Formation rigged up and bullheaded a 30 sack Class A Clinton cement plug weighing 15.6 pound per gallon from 5890 feet to 5552 feet squeezing 1 barrel away and displacing cement with 88.5 barrels of water. Appalachian Well Surveys (AWS) arrived, rigged up and tripped inside the 4.5 inch diameter casing with a 17 shot collar buster to shoot the casing off. AWS attempted to shoot the casing at 4904 feet but the gun failed to fire. AWS tripped out of the casing and found the equipment was shorted. AWS tripped inside the 4.5 inch diameter casing with a 4.5 inch diameter casing cutter and successfully shot the casing off at 4900 feet. AWS tripped out of the casing and BWS pulling 66000 pounds on the casing unsuccessfully trying to free casing up. AWS tripped inside the 4.5 inch diameter casing with a 4.5 inch diameter casing cutter and successfully shot the casing off at 4400 feet. AWS tripped out of the casing and BWS pulled on the casing successfully freeing it up. BWS will trip casing out of the wellbore to 4180 feet and complete plugging operations tomorrow. H2S was monitored throughout the day with no detection after the well was treated.

During my inspection the site had been roughly graded and all equipment had been removed. Two joints of 1.5 inch diameter pipe remain on location. The site will need to be final graded with vegetation established. The well was plugged on 2/28/18 and remains within the final restoration timeline requirements written within the Ohio Revised Code 1509.072 (B).

During my inspection the location had been graded and all production equipment and debris have been removed. Oil saturated soils were located were the pit was for plugging operations. The pit had been closed but the oil saturated soils remained. On 6/6/18 I contacted Corey McCudden with Northwood Energy and Mike Corp a contractor for Northwood to discuss the contamination issues. I contacted Mike Corp today 7/6/18 and Mr. Corp stated they planned on getting to the location next week.
