1392304996 2014-02-13 Production Wells Violations Noted On the day of my inspection, I reviewed the Division of Oil and Gas production records and found them to be current for the years of 2011 and 2012, the well is equipped with a pumping unit standing above an eight and five-eighths inch surface casing with four and a half inch production casing and two inch tubing. Upon inspection, the pumping unit was not operating. The pressure gauge on the two inch tubing was showing one-hundred and ninety pounds of pressure, and the gauge on the four and a half inch production casing was no longer readable. The valves on the two inch and the four and a half were both in the open position at the time of inspection. The valve on the two inch production line leading from the well was open, this line leads into a separator / drip / chart recorder and (2) one-hundred barrel production tanks sitting inside a containment dike. The valves were in the closed position at the separator and the drip on the production line. There was no I.D. posted at the well or at the tanks.

The requirements to meet the Division of Oil and Gas standards are as follows: 1.) Properly display identification information at the well and tank.

-39 and NOV-1392304996, I witnessed that the tank battery consisted of 2- 100bbl tanks within an earthen dike, a vertical separator, and a 2 inch meter run with a .25 inch orifice and chart recorder. Valves were in the closed position on the sales line. The well was capable of production and equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit powered electrically with the power in the off position. 2 of 4 valves were open on the casing and tubing with valves open on the flow line. Production records were current and identification was present at the well and the tank battery. Issues have been resolved.

1553795029 2019-03-28 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 2.375-inch diameter tubing, rods and a pumping unit powered. Production valves were in the open position. The production equipment consisted of two 100-barrel tanks, one vertical separator, one drip, and one sales meter. The sales meter had a 2-inch sales line, a .25-inch orifice plate, and a chart recorder with chart dated 3/27/2019. Identification was located on the chart and production records were current.
