0044764935 2016-07-18 Production Wells No Violations On the day of my inspection following up on Complaint ID4733, Compliance Notice-1216214585 and Compliance Notice-830050945, I witnessed that there was no change in the status of the well since last inspection on April 27, 2016. The production equipment consisted of a 100 barrel tank in an earthen dike. The well was equipped with 4.5 inch casing, a concoil catcher reduced to a 2 inch flow line with the valve in the open position going directly to the tank. The sales line has been disconnected and plugged with no separator or salesmeter present. No identification was present and gas production was reported to the State for 2015.
0189510909 2014-04-10 Production Wells No Violations On the day of my inspection following up on Chiefs Order #2014-39 and NOV-830050945, I reviewed The Division of Oil and Gas production records and found them not current for the years of 2012 and 2013. The well is equipped with four and a half inch production casing and a concoil catcher, the ball valve on the four and a half inch production casing appeared to be open, the valves were closed on the sales line, the lines to the production tanks were disconnected. The top is broken on the poly brine tank and the tank spacing does not meet the SPCC thirty six inch requirement, the I.D. is now in place.

-39, NOV-1216214585 and NOV-830050945, I witnessed that the tank battery consisted of 2-100bbl tanks, a 50bbl brine tank, a horizontal separator, a 2 inch sales meter with a .25 inch orifice, a chart recorder with no chart. Valves were closed on the sales line. Lines to the tanks were disconnected, top is broken on the polymer brine tank and tank spacing tank does not meet the 36 inch requirement. The well is equipped with 4.5 inch casing stubbed with a 4.5 inch x 5 foot pup joint plugged with no flow line to be found. Identification is now in place. The well remains incapable of production and tank spacing still does not meet the minimum requirement.

The well was equipped with 4.5 inch casing reduced to a new 2 inch flow line with the valve in the open position going directly to the tank 0psi was on the casing. Sales line has been disconnected and plugged. Polymer brine tank, separator and additional 100bbl tank have been removed resolving the tank spacing issue. Identification is not place at the tank or the well.

The sales line was disconnected and plugged with no separator or salesmeter present. No identification was present and production records were current with zero production reported to the State in 2016. A plug back permit was issued for the well on April 25, 2017 with Stonebridge proposing to have the well plugged back and producing if warranted by December 31, 2017. If plug back is not warranted the well will be plugged by December 31, 2017 and reclaimed according to ORC 1509.072B.

The production equipment consisted of a 100 barrel tank, a disconnected concrete vault, a horizontal separator and a 2 inch salesmeter with a .75 inch orifice and a chart recorder with no chart present. Valves were closed on the sales line. The well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, a plunger lift system and the valves were closed on the flow line and on the casing at the well head. Identification was posted on the tank and gas production was reported to the State for 2016. A plug back permit was issued for the well on April 25, 2017 with Stonebridge proposing to have the well plugged back and producing if warranted by June 30, 2018. If plug back is not warranted the well will be plugged by June 30, 2018 and reclaimed according to ORC 1509.072B.

1403191907 2014-06-18 Production Wells No Violations Prior to my inspection, I received a phone call from Rocky King the Administrator for the South East Region Division of Oil and Gas informing me of a complaint call on the well listed above. My inspection at the well site revealed the well is equipped with four and a half inch production casing / ball type gate valve with a steel two inch flow line and two inch ball valve leading into (1) one hundred barrel production tank. All the valves were open at the well head and on the flow line, the sales line has been disconnected and plugged with a steel bull plug, gas sales have been reported to The Division of Oil and Gas for the years of 2012 and 2013, no I.D. is posted at the well site.

1486062969 2016-11-29 Administrative Inspection No Violations Today, Mr. John Schneider, a Field Supervisor for Stonebridge Operating, returned my phone call from 11/28/2016. At 8:32 AM, I explained to Mr. Schneider that the Division had issued violations a few years ago on the P. Hall #1 and the Shriver #1 Wells located in Guernsey County. Inspections have shown that both wells currently remain idle. Mr. Schneider was asked if he had any interest in scheduling a meeting to inspect the wells together. Mr. Schneider agreed to meet with myself and Inspector Doug Bennett to inspect both wells. We agreed to talk again on Friday to discuss our schedules for the upcoming week.
1528200038 2018-06-05 Production Wells No Violations The purpose of my inspection was to follow up on a compliance notice numbered -1216214585. During my inspection the well was equipped with 4.5 inch diameter production casing and a ConCoyle casing lift system. The casing valve appeared to be in the open position and a 2 inch diameter sales line was attached to the well. The sales line is ran to the 100 barrel tank and line appeared discontinued with a solid plug at the end of the line. There was not any gas production equipment or sales meter present. No identification was located in a conspicuous space on or near the wellhead or the 100 barrel tank. Production records have been submitted with Stonebridge Operating Company reporting zeros for production throughout the years 2016 and 2017. Pictures were taken to assist this inspection.
1539260116 2018-10-10 Plug / Plug Back No Violations

-inch tubing into 3737 feet. Cemenco pumped a 60--inch and ripped the casing at 1507 feet. The 4.5-inch diameter casing was tripped from hole. The 2.375-inch diameter tubing was tripped in hole to 1364 feet and Cemenco pumped 200-foot plug 100 feet in the 8.625-inch diameter surface casing and 100 feet out in the 7.875-inch diameter borehole. The tubing was tripped up and a 100-foot surface plug was pumped with 30 sacks of class A cement. The cement stayed at surface. The plugging of this well satisfies compliance notice -1216214585.

The well is not capable of production with only 4.5 inch casing stubbed with a 4.5 inch x 5 foot pup joint plugged with no flow line to be found. No identification was present and the production records for 2012 meet the minimum requirements. Well shall be plugged.

Mr. Schneider stated that in the current condition of the well it could not be producing any gas. Mr. Schneider also stated that he would draft a plan to address the status of the well and submit the plan to ODNR within two weeks.
