0516863682 2016-12-01 Production Wells No Violations On the day of my inspection following up on Complaint ID 5483, I witnessed the well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing, rods, an American pumping unit with no motor present. Valves were closed on the tubing and 1 of 2 open on the casing a reduced to a 1 inch diameter plastic flowline supplying domestic gas. The access road also serves as a driveway to the residence of 5628 Bloomfield Road shows minor erosion near the top but is drivable. The production equipment included a 100 barrel tank within an earthen dike. Also present near State Route 209 a gravity fed 100 barrel transfer tank within an earthen dike at GPS coordinates 40.047445 -81.632576. Identification was present legible but faded. I placed call to Deer Creek and spoke to Ervin Morris and I asked where the gas was being going since there was no separator and meter onsite. Mr. Morris stated that the gas is being used at 5628 Bloomfield Road and at their shop on State Route 658. I also stated that they need to keep an eye on the road to make sure no more erosion takes place. Mr. Morris stated that they plan on dressing road up when it is dry enough.

2074669939 2015-12-17 Production Wells Violations Noted On the day of my inspection following up on Complaint ID 5120, I witnessed the well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, rods, an American pumping unit powered by natural gas with valves in the open position. The production equipment included a horizontal separator, a drip, a 2 inch sales meter with a .25 inch orifice and a chart recorder, a 100bbl tank. Also present near SR 209 a 100bbl transfer tank within an earthen dike at GPS coordinates 40.047445 -81.632576. Identification was present but not legible. A 3 foot diameter area around the wellhead was contaminated with oil along with contamination present underneath the natural gas motor. The separator did not meet the setback distance of 50 feet from the wellhead. The 100bbl tank on location was missing a 2 inch plug and leaking resulting in a 3 foot by 50 feet area of stressed vegetation down the side of the location. The 100bbl tank near SR 209 was missing a 2 inch plug. Owner/operator shall post identification in accordance with OAC 1501:9-9-05 (A) (10), remove all contamination, replace with clean fill and dispose of properly and properly dispose in a landfill and provide ODNR with the manifest for the disposal. Owner/operator shall construct a SPCC dike and install plugs or locks where instructed. Owner/operator shall move separator in accordance with OAC 1501:9-9-05 (A) (5). I placed call to the number on record and spoke with Susan Bates who sent out well tender Jim Jones. Mr. Jones was instructed to remove contamination and make repairs as set forth in violation. Mr. Jones stated that work would begin tomorrow.
