0388556281 2017-12-12 Production Wells No Violations On the day of my inspection following up on Compliance Notice 1506619852 & Complaint ID 5712, I witnessed the production equipment included 2-210 barrel tanks and a disconnected 210 barrel tank all within an earthen dike and shared with 34059213420000 Fehrman 1. Also present near the dike an Oilwell pumping unit. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing. 2.375 inch diameter tubing and rods with all valves in the closed position. Communication received today from Pin Oak states that they will have the well plugged by June of 2018.
0714378288 2019-03-28 Production Wells No Violations The purpose of my inspection was to follow up on Compliance Notice 1506619852 and Complaint 5712. The compliance notice stated that the well shall be plugged. During my inspection the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, and rods. No pumping unit was attached. All production valves were in the closed position. Following my inspection, I called Allan Berry with Berry Enterprises to discuss the plans for the well. Mr. Berry stated that he will be bringing a service rig to service the well and bring it back into production. The service rig is expected to arrive within the next two months.

Call was placed to Mark Van Tyne of Pin Oak requesting an update. Mr. Van Tyne stated that a rig would be onsite late spring.

No pumping unit was attached. All production valves were in the closed position. Production reports indicate zero production for years 2016 and 2017.

Also present near the dike an Oilwell pumping unit. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing. 2.375 inch diameter tubing and rods with all valves in the closed position. Identification was present and production records were not current with the last reported production in 2010 by the last owner of record. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
