0545144446 2013-04-10 Production Wells No Violations On the day of my inspection I reviewed the Divisions production records and found they were current, there is a pumping unit at the well and all the valves are open, there is a separator/ drip/ chart/ compressor setting out side of the containment dike, there is (1) one hundred barrel production tank and (1) fifty barrel plastic brine tank setting inside the containment dike, the I.D. is posted on the tank.

1275163929 2010-05-28 Production Wells No Violations Received phone call from local authorities concerning lightening striking oil tank and tank exploded releasing contents within dike area. Upon arriving on site along with local fire departments found tank on fire and contents running toward local creek, we contained oil before it entered creek up until the point that water and foam forced flames into creek bed .30-40 bbl oil was thought to have been in tank at time of storm, stayed on site until fire was out, called Dennis Deaver of Ohio EPA and informed him of accident and what clean up efforts were being applied at scene, will check on site tomorrow and check creek and dike area for oil and witness clean up efforts.

1502716391 2018-03-14 Production Wells No Violations On the day of my inspection I witnessed the production equipment included horizontal separator, a compressor unit, a 2 inch meter run with a .375 inch orifice and chart recorder. Also present a 100 barrel tank and 50 barrel plastic tank both within an earthen dike. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods and a National pumping unit powered by electricity with valves in the open position. Identification was present and production records were current.
1593904961 2018-08-20 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625-inch diameter casing with a casing head, 4.5-inch diameter casing with a tubing head, 2.375-inch diameter tubing, rods and a pumping unit. Production valves were in the open position. The production equipment consisted of one 100-barrel tank, one 50-barrel poly tank, one horizontal separator, one compressor, and a sales meter. The sales meter had a 2-inch diameter sales line, a .375-inch orifice plate, and chart recorder. Identification was present and production records were current.
