The conductor was set on 8/31/12 and the drilling permit expired on 6/21/14. A plug permit was issued on 8/18/17 and multiple attempts were made contacting the owner to discuss plugging of the well with no response. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The well owner/operator shall also plug the mouse hole in accordance with OAC 1501:9-11-11(A). Upon plugging the well owner/operator shall complete restoration according to ORC 1509.072(B). |

NWP pumped 126 sacks of Class A cement weighing 15.6 pound per gallon from 66 feet to surface circulating cement to surface. NWP tripped 1.5 inch diameter tubing out and back in the 14 inch diameter mouse hole tagging again at 66 feet. NWP pumped 65 sacks of Class A cement weighing 15.6 pound per gallon from 66 feet to surface circulating cement to surface resolving the notice. Operator was instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B. |

During my inspection the conductor and mouse hole was found to be plugged and abandon. The pad was left empty with no production equipment, only some small steel debris centering the pad. The lease road and the pad had been left as constructed. The pad appeared to be stable with no visible slip failures or extensive erosion. Gravel had been left covering the lease road and the pad. On June 1st, 2018 a landowner waiver had been completed and signed by the land owners Danny Brill, Deborah Brill, and the company Devon Energy. The landowner waiver stated that Devon Energy is not responsible for any restoration work. As written within the Ohio Revised Code 1509.072(B) the completed waiver shall release Devon Energy from all restoration work, therefore, the location has met Final Restoration requirements. |
