0801661024 2013-05-15 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Timco Wire line Services perforate the seven inch surface casing at 690 with six .375 shots, at that point Formation Cementing made an attempt to circulate water but would not circulate. A decision was made to perforate the seven inch at 530 with six shots, still no circulation. I made a phone call to the South East Regional Supervisor making then aware of the problem with the seven inch surface casing, the decision was made to perforate at 275 and if no circulation was established it at that point was a lost hole.
1368486060 2013-05-13 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed cement returns on the 9.625 conductor, the cement stayed up to the top, the pit was constructed and had the liner installed through the flow ditch to the well head.
1407423394 2014-08-07 Preliminary Restoration No Violations On the day of my inspection I witnessed that the production equipment included 2-100bbl tanks, a 30bbl brine tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit powered by natural gas with valves in the closed position at the wellhead. Well identification was present and the site passed preliminary restoration according ORC 1509.072A.

1659203868 2013-05-14 Administrative Inspection No Violations County inspector called on well that had no returns of cement back and was circulating. Upon displacing the plug down the well the displacement stops. The inspector calls and I recommend to shut down and let the cement cure. Then let the drill rig drill out down to the bottom of the 7casing and bond log hole. Then perforate and squeeze cement. Then go in check it if need to with a second bond log.
