0499188191 2014-08-07 Preliminary Restoration No Violations On the day of my inspection I witnessed that the production equipment included 2-100bbl tanks, a 30bbl brine tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit powered by natural gas with valves in the closed position at the wellhead. Well identification was present and the site passed preliminary restoration according ORC 1509.072A.
1380462864 2013-09-29 Drill / Deepen / Reopen No Violations I was notified that Northeastern would be cementing surface casing at 8:30am and I arrived on site today and witnessed that Northeastern Drilling Rig #4 had drilled an 8.875 hole to a depth of 1650. While running casing they got hung up at 650 and Eclipse made decision to pull casing and trip back in the hole with drill pipe. Tool pusher would notify me once they got pipe back in the hole for cementing. No violations at this time. When pulling casing out the hole they lost part of a centralizer. Blue Dot was on their way to the site to fish. Blue Dot made 3 runs and retrieved some portions of the centralizer. Northeastern tripped back in the hole with their 8.875 bit and reamed the hole down to 1643. Blue Dot was going back in to TD to fish out remainder of the centralizer.
