Gauges present read 2910psi on the tubing and 3050psi on the casing with annular pressure monitoring in place.

Also present for the well was a disconnected line heater, a GP unit and a sand trap. The well was equipped with a production tree and with the valves in the open position. Gauges present read 235 pounds per square inch on the tubing and 430 pounds per square inch on the casing with annular pressure monitoring in place. The annulus between the 9.625 inch diameter casing and the 5.5 inch diameter casing was at 1160 pounds per square inch. Installation of valves is now in place to manually selloff annular gas.
1059156477 2015-02-12 Drill / Deepen / Reopen No Violations On the day of my inspection I received phone call from Drilling Superintendent David Newman at 20:17 on 2/12/15 stating that they tripped in and drilled out float equipment and tried to circulate the wellbore and was unable. At that time the plan was to trip out of hole and re-install landing joint and try to gain circulation. I received second call at 22:09 that they had gained circulation and was preparing to cement the intermediate string. I stated that a casing integrity test would be required due to the float equipment being drilled out prior to cementing. Schlumberger gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 890 sacks of Class A 15.6ppg, 1.23cuft/sack cement. Cement returns of 50bbls to surface was reported. Upon arrival for test Nomac Drilling Rig #72 had drilled out cement and was preparing for FIT. Nomac Drilling Rig #72 pressured up to 840psi with zero drop in pressure after 30 minutes.
1098288322 2015-01-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Express Energy Services had drilled a 30 inch diameter hole to the depth of 120 feet hitting water at 105 feet and ran 120 feet of 24 inch conductor pipe. Express gained circulation and cemented pipe with 230 sacks Class A cement with 2% CaCl and .5#/sk. of floseal. Displaced cement with 39bbls of fresh water and had good cement returns of 15bbls to surface.
1104845511 2016-09-15 Production Wells No Violations On the day of my inspection I witnessed the production equipment included 12-400 barrel tanks within containment, 2-NGL tanks, a vapor recovery tower, 2 low pressure horizontal heated separators, a vertical volume pot, 2-oil stabilizer units and a gas combustor all shared with the 1H, 3H, 5H, 7H, 8H, 9H and 10H. Also present for the well was a line heater, a GP unit and a sand trap. The well was equipped with a production tree and with the valves in the open position. Gauges present read 1200 pounds per square inch on the tubing and 1520 pounds per square inch on the casing with annular pressure monitoring in place. I met with lease operator Corey Shriner as annular pressure on the 9.625 inch casing was at 1132 pounds per square inch. Mr. Shriner relieved the pressure to 150 pounds per square inch and was instructed to keep pressure under 741 pounds per square inch to be compliant with OAC 1501:9-1-08 (N).

Also present for the well was a line heater, a GP unit and a sand trap. The well was equipped with a production tree and with the valves in the open position. Gauges present read 275 pounds per square inch on the tubing and 495 pounds per square inch on the casing with annular pressure monitoring in place. The annulus between the 9.625 inch diameter casing and the 5.5 inch diameter casing was at 1172 pounds per square inch. I placed call to Chesapeake Regulatory Affairs Advisor Jody Jones and discussed annular pressures. Mr. Jones stated that he is having a meeting later this week with ODNR to discuss annular issues and that he would get back to me with their plan to address. Identification was present and production records were current.
1424785098 2015-02-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Nomac Drilling Rig #72 had drilled an 8.5 inch diameter lateral hole to a depth of 15981 feet and 5.5 inch diameter P-110-HC 23#/ft. casing was set at 15981 feet. Schlumberger had cemented casing with 840 sacks of Class A 13.5ppg, 1.68 yield with adds lead cement followed by 1840 sacks of Class A 15.0ppg, 1.30 yield with adds tail cement. Cement was displaced with 355bbls of fresh water with not landing the plug an additional 15bbls of fresh water with no increase in pressure. Pressure was held on casing at 1130psi with no drop for 5 minutes with the floats holding returning 3bbls back to the tubs when released. Cement was seen at the surface with the calculated top being 65 feet from surface. Chesapeake was instructed to complete casing test when the rig arrives on site for completion.

The production equipment consisted of twelve 400 barrel tanks with one vapor recovery tower, eight sand traps, eight gas production units, two heated horizontal separators, and one combustor. Identification was present and production records were current. A Chesapeake contractor was on location preforming maintenance on the gas production unit.

Also present for the well was a GP unit and sand traps planned for installation are currently not on site. The well was equipped with a frac valve in the closed position and drill out is scheduled to begin the week ending 3/26/16.
1865213790 2015-02-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Nomac Drilling Rig #72 drilled a 12.25 inch diameter hole to the depth of 2458 feet. Wyoming Casing Crew ran and set 2448 feet of 9.625 inch 40#/ft. J-55 casing. Nomac Drilling Rig #72 began pumping on casing with their rig pumps to break circulation at 146bbls in the casing pressured up to 3000psi. The rig broke their lines to check for blockage and pumped on it again and it pressured up for the second time. Schlumberger then attempted to pump on casing resulting in it pressuring up for a third time. Chesapeake made decision to lay casing down and find the problem and rerun casing with cementing to follow.
