0242246806 2016-02-29 Production Wells No Violations On the day of my inspection I witnessed that the location was gated and locked. The production equipment included 12-400bbl tanks within containment, a vapor recovery tower, a vapor blow case, a vertical 3-phase separator, 2 compressor units, a vertical heater treater, a 50bbl sand tank within containment, a gas combustor and back-up flare stack all shared with the Karen 23-9-7 1H, Karen 23-9-7 2H and Karen 23-9-7 3H. Also present a horizontal line heater, a vertical separator and a sand trap. The well was equipped with a production tree with valves in the open position with 1000psi on the tubing and 1100psi on the casing. Annular pressure monitoring was present along with identification and production records were current.

During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. A gauge was present reading 480 pounds per square inch on the 9.625 inch diameter intermediate casing. The production equipment consisted of one 500 barrel tank, eleven 400 barrel tanks, one 50 barrel tank, one vapor recovery tower, four sand traps, four line heaters, four gas production units, one heater treater, one compressor, one vertical vent scrubber, and two combustors. No personnel on location during inspection.

1111620122 2014-04-22 Drill / Deepen / Reopen No Violations At 5:13 PM I received a call from Chris Anderson, a consultant for Gulfport Energy. Mr. Anderson stated that a fire had occurred at the well site at 2:30 PM and that the Quaker City Fire Dept. was dispatched to put it out. I drove to the well site to photograph the damage and asses the impact. My inspection found the fire originated in a Sandex tank unit. The heat from the fire caused the containment around the tank to catch on fire. There were no indications that any fluids leaked from the tank. ECM was on site removing the remaining fluid from the Sandex tank. I instructed the company representative to remove all stone with melted plastic on it and repair the containment. I recommended that the company review their tank grounding protocols.

1385498015 2013-11-26 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed O-Tex Pumping LLC break circulation with 113.29 Bbls of mix water and displace 300 sacks of 50/50/ POZ Class H cement for the lead cement, then with 373.91 Bbls of mix water displace 2760 sacks of Premium H cement. I witnessed O-Tex bump the plug, the final lift pressure was 1900 lbs of pressure, the floats held and total circulation was 1128 Bbls.
1767337724 2013-12-13 Production Wells No Violations On the day of my inspection, I witnessed W. Pol Contracting making well site preparations to the pad so temporary containment can be put in place for the frac tanks and equipment, the transfer of ownership from Chesapeake to Gulfport Energy has been submitted ODNR Central Office and waiting for approval, I instructed Gulfport Energy they could do site preparation to the pad but are not to stimulate or do any down hole work to any of the wells on this pad until the transfer of ownership has been established.
