The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 320 pounds per square inch on the tubing and 510 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current.

The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 290 pounds per square inch on the tubing and 475 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current. An Above Ground Storage Tank (AST) was present on the northwest corner of the well pad supplying fresh water to the Frank Miller pad for frac with support being provided by Great Lakes. Water was field tested revealing chlorides of <33 parts per million, pH 7 and 60 degrees.

The production equipment included a sand trap; a GP unit and 2-400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 1040psi on the tubing and 1270psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. We completed a walk around with Eclipse Well Tender Bradley Paisley.

1059776683 2018-06-11 Production Wells No Violations The purpose of my inspection was a status check The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 636 pounds per square inch, 1141pounds per square inch on the production casing, 500 pounds per square inch on the 9.625 inch diameter intermediate casing, and 0 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of two 400 barrel tanks, six sand traps, six gas production units, and one indirect line heater. An above ground storage tank was situated in the Northwest corner of the pad. The identification was located at the wellhead.

Gauges present read 1660psi on the tubing and 1870psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. Discussion was held on the readings with Eclipse Lease Operators Bradley Paisley and Danny Hreha.
1305517155 2015-03-02 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well was equipped with Sweco wellheads and production trees. The production equipment included a sand trap, a GP unit and 2 400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Tubing and Casing pressure was at 3000psi with the intermediate and surface casing at 0psi. The site passed preliminary restoration according to ORC 1509.072A.

Gauges present read 750psi on the tubing and 800psi on the casing.
1393366640 2014-02-24 Drill / Deepen / Reopen No Violations On the day of my request inspection, I witnessed Alpha Hunter Rig #9 finish running the 18.628 surface casing to a total depth of 470, Schlumberger broke circulation with fresh water then displaced 545 sks of Class A cement, I witnessed 43.0 sks of cement returns to surface.
1393433997 2014-02-26 Casing String Witnessed No Violations On the day of my inspection, I witnessed Wyoming Casing Crew running the last four joints of 13.375 intermediate casing to the required depth of 1550, at 1487.9 the casing was hung in the hole, the rig crew for Alpha Hunter #9 and Wyoming casing crew along with Schlumberger circulated the hole while attempts were made over a period of eight hours to meet the minimum requirements, the casing would only drop two feet, I approved the decision to cement from that point back to surface.
1407339533 2014-08-06 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that Nomac Drilling Rig 359 was drilling an 8.5 inch diameter lateral hole at a depth of 13001 feet with a planned td of 15200 feet. I met with Company Man Jeb Dobbs and completed walk around. Site inspection revealed some erosion on the southwest side of location. I made a call to Bruce Carpenter of Eclipse and advised him of my findings and needed a plan to address it.
1438130230 2014-02-26 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Schlumberger break circulation with fresh water, then mix and displace two hundred and fourty six sacks of Class A cement with a weight of 15.6 and a yeild of 1.20, I witnessed fourty seven barrels of cement returns to surface, Schlumberger applied five hundred pounds of pressure against the plug and held for five minutes.
