0379754981 | 2016-09-29 | Production Wells | No Violations | On the day of my inspection I witnessed the well was equipped with a plunger lift system with valves in the open position. Gauges present read 665 pounds per square inches on the casing and 495 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater, a GP unit and 2-400 barrel purge tanks within containment and shared with the 8H, 6H and the 2H. Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF for sales. |
0411055543 | 2016-03-21 | Production Wells | No Violations | On the day of my inspection I witnessed that the production equipment included a line heater ,a sand trap, a GP unit and 2-400bbl purge tanks. The well was equipped with a plunger lift system with gauges present reading 830psi on the casing, 600psi on the tubing and valves in the open position Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF. |
0458907569 | 2015-04-07 | Production Wells | No Violations | On the day of my inspection I witnessed that the production equipment included a sand trap, a GP unit and 2 400bbl purge tanks with the well online and 2100psi on the tubing and 2300psi on the casing. Site inspection of the Hayes Central Gathering Facility revealed that Eclipsed has implemented the plan to address concerns of the flare stack and the loading of NGL trucks. Once Eclipse receives their air permit they will run a second compressor reducing the size of the flare. |
0793946651 | 2015-01-27 | Preliminary Restoration | No Violations | On the day of my inspection I witnessed that the production equipment included a sand trap, a GP unit and 2 400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. The site passed preliminary restoration according to ORC 1509.072A. |

1325556909 | 2018-07-19 | Preliminary Restoration | No Violations | The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a plunger lift system with valves in the open position. Pressure gauges for all casing strings were present and in plain view with a casing identification tag. The production equipment consisted of two 400-barrel tanks, four sand traps, and four gas production units. The location meets Preliminary restoration requirements written under ORC 1509.072 (A). |
1548264240 | 2014-06-30 | Drill / Deepen / Reopen | No Violations | On the day of my inspection accompanied by ODNR Inspector Mike Hartshorn, following a request to verify Eclipse Resources was properly pulling water from a permitted source, I witnessed that there was a pumping station at the coordinates of 39.969217/-81.316833 which was insulated to provide a sound barrier to neighboring homes and was pulling water from the Leatherwood Creek which is part of the Ohio River watershed and permitted to withdrawal an estimated 30,000 gal/day and an estimated total withdrawal of 7,000,000 gallons. A second inline pump was located at the coordinates of 39.976438/-81.319556 and a third inline pump located at the coordinates of 39.992704/-81.315662. All pumps were in containment and monitoring of intake was present and no leaks were noted. The line lead to an impoundment located at 39.989540/-81.316641 where I met with GRE General Manager Jeff Terrell and discussed why we were present and noted that outtake was being monitored as well. |
1603699364 | 2018-06-11 | Production Wells | No Violations | The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring for all casing strings were present. The production equipment consisted of two 400 barrel tanks, four sand traps and four gas production units. The identification was located at the wellhead. |
