0283253407 2018-07-24 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. A pressure gauge was present on all casing strings along with a pressure relief valve on the annulus. The production equipment consisted of six gas production units, one combustor, and fifteen 400-barrel tanks. A triethylene glycol dehydration unit with a contact tower and reboiler was located by the tank battery. The identification was located on the wellhead.

Mr. Smith stated that the work would be completed by the end of the week.

The site passed preliminary restoration according to ORC 1509.072A.

1403318867 2014-06-20 Drill / Deepen / Reopen No Violations On the day of my site inspection, there was no erosion and good vegetation established the lease road was in proper condition, all the chemicals and tots were setting inside containment, I witnessed Halliburton break circulation with fresh water, then mix gel and displace followed by Halcem cement with a weight of 15.6 and a yield of 1.22, there were forty barrels of cement returns to surface, Halliburton bumped 500 pounds of pressure on the plug and held for five minutes.
1405362911 2014-07-14 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Mike Hartshorn, I witnessed that Sidewinder Drilling Rig #63 had set a plug at 1080 feet. Halliburton was on site preparing to squeeze the intermediate casing with perforations at 1077 feet due to failure of the pressure test.

The identification was located on the wellhead.
