0075557225 2014-09-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Sidewinder Drilling Rig #63 had completed cementing their intermediate casing and was preparing to trip in the hole and drill out the dv tool and then test casing. Walk around with Company Man Aaron Wilhite revealed no violations.
0227865630 2014-10-03 Casing String Witnessed No Violations I was onsite to witness the cementing of the surface casing. Sidewinder rig #63 ran 31 joints of 54.5# J-55 STC 13 3/8 casing, tagged fill at 1298, hooked up the lines and washed/cleaned the fill from 1298 and set the shoe at 1308, hooked up Halliburton cement equipment, pumped 50 bbls of gel ahead followed by 5 bbls of freshwater spacer then mixed and pumped 234 bbls of cement slurry weighing 15.6 ppg using 1080 sacks of Halcem cement + additives, released the top plug, displaced cement with 196 bbls of freshwater, bumped the plug at 02:00 hrs with 1,000 psi, released the pressure, floats held. Circulated 5 bbls of good cement to surface with no fallback. Cleaned up, rigged down Halliburton, removed the flowline and prepared to nipple up while waiting on cement.
0258948207 2014-09-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Sidewinder Drilling Rig #63 had ran a temperature/bond log and found that top of cement was at 118 feet. Halliburton was on site and 1 inch pipe was ran and tagged up at 118 feet. Halliburton pumped 125 sacks of Class A cement with a 15.6ppg density and a yield of 1.18 and cement came up to 80 feet. Sidewinder Drilling Rig #63 pulled 2 joints of 1 inch pipe and mixed a LCM pill and Halliburton pumped 125 sacks of Class A cement with a 15.6ppg density and a yield of 1.18. Sidewinder Drilling Rig #63 mixed another LCM pill and Halliburton pumped 125 sacks of Class A cement with a 15.6ppg density and a yield of 1.18. Had good cement returns to surface.

Sweco arrived on location and found a faulty filter on a sensor and it was replaced and the rig went back to tripping out to change mud motors

I placed call to REX Energy EHS Supervisor Derrick Smith and I stated that valves were in place for annular monitoring but no gauges or relief valves. Mr. Smith stated that the work would be completed by the end of the week.
1410105724 2014-09-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Sidewinder Drilling Rig #63 drilled a 17.5 inch diameter hole to the depth of 1315 feet. Franks Casing Services ran 1315 feet of 13.375 inch J-55 54.5#/ft. Halliburton was on site and gained circulation with 150bbls of fresh water then pumped 50bbls of gel, 5bbls of fresh water and cemented casing with 1025 sacks of Class A cement with a 15.6ppg density and a yield of 1.21. Displaced cement with 195bbls of fresh water. There was no cement returned to surface and plug was landed at 500psi and pressured up to 1000psi for 5 minutes and the floats held. Rex Energy will be running a temperature and bond log to determine TOC and remediation.
1419343118 2015-02-25 Preliminary Restoration No Violations On the day of my inspection I witnessed that flow back has been completed and the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a GP Unit, an oil stabilizer unit, a vapor recovery system and a total of 15-400bbl tanks all within containment. The site passed preliminary restoration according to ORC 1509.072A.
1480124958 2018-07-24 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. A pressure gauge was present on all casing strings along with a pressure relief valve on the annulus. The production equipment consisted of six gas production units, one combustor, and fifteen 400-barrel tanks. A triethylene glycol dehydration unit with a contact tower and reboiler was located by the tank battery. The identification was located on the wellhead.
