1354133437 | 2015-07-21 | Production Wells | No Violations | At 4:17pm I called Production Supervisor Paul Arblaster and asked if they had a fire at the J Hall or the J Anderson and he stated that they did at the J Hall and it was out as he was just arriving at the site. Upon my arrival at 5:07pm I met with Mr. Arblaster and Construction Supervisor Jim Taydus and completed walk around discussing the incident. Mr. Titus stated that Jennings Contractors: Jason Matthews, John Lowe and Cody Dison were disconnecting a temporary flare stack along with the piping and preparing to install a gas combustor. Mr. Dison was hitting a 3 inch hammer union which was isolated and going to the old flare stack when the threads came apart on a live 4 inch Vapor Recovery Line which was upstream at a tee resulting in vapors being released and then ignited by the Profire control box which controls the burners on the GP Unit for the J Hall 2H. All 4 men were accounted for and then Mr. Taydus shut the 6 wells in and called 911 at 2:40pm. Mr. Taydus accessed the fire, the wind and proceeded to extinguish the fire with a single fire extinguisher and then he shut the vapor recovery system down. Jennings contractors met with the Quaker City Fire Department at the entrance to the well pad and told them that the fire was out and they were not needed. Repairs were made to the Vapor Recovery Line and the 5 of the 6 wells were put back online at 6:10pm with the 2H being shut-in until repairs to the Profire control box could be completed. I placed call to REX Energy HSE Supervisor Derrick Smith and stated that when they have an incident on one of their sites that ODNR needs to be notified and he stated that it was his fault that ODNR was not notified and that proper notification would be made in the future. |
