0121690801 2018-07-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection Deep Well Services Rig 4 was rigged up on the Yost B5H. I spoke with Eclipse Company man and he stated that they were in the process of installing a plunger lift system. Two 500-barrel frac tanks and one 500-barrel gas buster were being utilized. All casing strings were equipped with a pressure gauge and were being monitored. The area effected by the construction of the pad has been restored with vegetation established. The location meets preliminary restoration requirements listed under ORC 1509.072(A).

Ohio-West Virginia Excavation is onsite setting cellars for the Great Scott #3H, Outlaw A #11H, Yost A #1H and Yost B #5H.

During my inspection the well was equipped with a plunger lift system with valves in the open position on the production tubing. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, the 5.5-inch diameter production casing had 871 pounds per square inch, and the 2.875-inch diameter tubing had 500 pounds per square inch. The production equipment consisted of eight sand traps, eight gas production units, and three 400-barrel tanks. The produced water was being separated on pad and distributed in the 400-barrel tanks. The oil and gas were being gathered at the Eclipse Hayes CGF pad via pipeline. Identification was located at the wellhead.

1408465967 2014-08-18 Casing String Witnessed No Violations I was onsite to do my daily inspection and to witness the cementing of surface casing. Nomac 359 has drilled a 24inch hole in diameter to a depth of 441ft. A total of 18.625inch 87.5# casing was ran in the hole to a depth of 435ft. Halliburton used 800sks at 15.6# to surface for the cement job. We received 15bbls cement returns to the surface.

1894522850 2014-08-28 Drill / Deepen / Reopen No Violations During the time of my inspection Nomac 359 were tripping the drill pipe back in the hole. They were at a depth of 6,789ft. They will be drilling out the 9-5/8 float. The wall on the backside of the location has started to fall in from the rain. On 8/30/2014 company will be shutting down and moving there equipment to fix the wall.
