0620266560 2015-07-16 Completion Testing No Violations On the day of my inspection I witnessed that flowback testing was completed and production equipment including a sand trap, a line heater, a GP Unit along with 2-400bbl purge tanks within containment was present and construction was 90% completed. The well was equipped with Sweco wellheads and production trees with valves in the closed position and proper identification present. Fluids and gas will be separated and transferred to the Borton CGF for further processing when the well is opened and online.

0828253773 2015-03-27 Completion Testing No Violations On the day of my inspection I witnessed the cellar fluid level was 8 inches from the surface with a light sheen present and mouse holes were not plugged and full of water. Field testing water reveled chlorides of 96ppm and a pH of 6. I placed call to Matt Amos of Eclipse and he stated that a water truck would be out immediately to pump the cellars and mouse hole. He also state that he would coordinate with Kyle Bradford to get the mouse holes plugged and would notify ODNR prior to cementing.
