H&P Flex Rig #385 ran 12889 feet of 5.5 inch casing and Universal Well Services cemented production casing. H&P Flex Rig #385 was beginning to rig down with plans to be stacked out until February.

During my inspection the well was equipped with a plunger lift system with production valves in the open position. Gauges were present reading 150 pounds per square inch on the tubing, 530 pounds per square inch on the production casing, 0 pounds per square inch on the 13.375 inch diameter intermediate, and 0 pounds per square inch on the 9.625 inch diameter second intermediate casing. The production equipment consisted of sixteen 500 barrel tank, four vapor recovery towers, four gas production units, one gas conditioning skid, one heater treater, one compressor, one vent scrubber, two combustors, and a Triethylene Glycol dehydration system consisting of one glycol contact tower and reboiler.

0950491272 2015-06-19 Production Wells No Violations On the day of my inspection accompanied by Regional Emergency Operations Manager Trisha Cartmell, we reviewed the incident that occurred on January 1, 2015 where a pickup belonging to Dynamic Structures which is a division of Clear Creek Oil Field Solutions caught fire on location due to a faulty fan resulting in total loss of the vehicle with no injuries. Casing pressure was at 2400psi and tubing pressure was at 2100psi with valves in the open position.

1425489316 2015-03-04 Completion Testing No Violations On the day of my inspection I witnessed CalFrac Well Services was on site fracing stages 6-10 out of a total of 41 stages through flow shoes and releasing frac balls between stages with treating pressure at 7689psi and a rate of 30bbls/min. Single stage frac to commence on stage 11-41 with perforating, frac and wireline plugs set on individual stages.

1436746017 2015-07-12 Production Wells No Violations While traveling West on I-70 in Wills Township in Guernsey County at approximately 5:20 pm I observed heavy black smoke and fire from the vapor recovery stacks located on the PDC Cole production pad. 911 was notified along with my supervisor Brent Bear and County Inspector Doug Bennett. I arrived on scene at 6:05 pm to find the fire had extinguished itself without spreading to the nearby tank battery and 3 local Fire Departments on scene. Inspector Bennett had made contact with PDC and I did receive notification from a Tony Long that a pumper was enroot. At 6:45 Pm a representative from PDC arrived and the situation accessed. At 7:45 pm a Blake Roush with PDC had also arrived on scene and updated the responding personnel of the status indicating that hydrocarbon product had entered the vapor recovery unit and had burnt. The lines were evacuated from additional product that was in the system rendering the system once again safe allowing the responding personnel to be released. There was a Ohio State Highway Patrol Supervisor on site.
1466458817 2015-01-01 Drill / Deepen / Reopen No Violations On the day of my inspection at 9:31AM I received a phone call from PDC Safety Director Tony Long stating they had a pick-up truck on fire on the Cole Pad. Upon arrival at 10:30AM I met with Old Washington Fire Department Chief Heath Beal who stated when they arrived that the pickup was completely engulfed and they used water and foam to extinguish the fire. The Chief stated that he could not determine the cause due to the extent of damage to the vehicle but appeared to have started in the engine compartment. During the fire a 70gallon diesel tank ruptured on the back of the truck releasing 1.5bbls of fuel onto the pad. The vehicle was driven by Randy Daniels of Dynamic Structures which is a division of Clear Creek Oil Field Solutions and they were on site preparing to power wash the remaining frac tanks on location. Mr. Daniels stated that they were inspecting the frac tanks when they saw smoke at 9:20AM they notified PDC Company Man Larry Howell who then called 911. Clear Creek Oil Field Solutions was given the approval that it was safe from the Chief and they began pulling up contaminated fluid around vehicle up. No injuries or damage to the wellheads and all fluids dispersed were inside containment.

1605694854 2014-12-14 Drill / Deepen / Reopen No Violations On the day of my inspection following up on a reported spill of 30bbls SOBM in the cellar, I witnessed that H&P Flex Rig #385 had landed their curve and began drilling their lateral. While reaming out their curve the rubber inside the Pruitt rotating air head failed resulting in the loss of 30bbls of SOBM with the mud going into the cellar and splashing onto the sub. Trash pumps were used to pump the mud back into their pits and rig hands power washed the sub pumping the residual to be disposed of properly.
1636765138 2014-08-17 Casing String Witnessed No Violations On the day of my inspection, I witnessed that First Class Drilling Rig #6 had drilled a 24 inch diameter surface hole to the depth of 544 feet and ran 544 feet of 20 inch 94#/ft. casing. Universal Well Services was on site and gained circulation with 25bbls of gel, 5bbls of fresh water and cemented casing with 725 sacks of Type 1 cement with 3% CaCl and .25#/sk. of cello flake. Had good cement returns of 25bbls to surface. The pressure test did not hold. They landed at 225psi and went up to 550psi but the plug did not hold. The head was shut in and the rig will pressure up on it in 8hrs.

BOP stack has been moved and preparations being made to begin testing.
