During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present with both intermediate casing strings reading zero pounds per square inch. The production equipment consisted of sixteen 500-barrel tank, four vapor recovery towers, four gas production units, one gas conditioning skid, one heater treater, one compressor, one vent scrubber, two combustors, and a Triethylene Glycol dehydration system consisting of one glycol contact tower and reboiler.

1191068681 2015-08-17 Production Wells Violations Noted On the day of my inspection I witnessed the GP Unit had a leak on the output oil line creating a 3 foot diameter area of contamination. I placed call to PDC District Operations Manager Blake Roush and stated that the GP Unit had a leak and Mr. Roush stated that a tender will be in route to shut well in and clean up contamination and make repairs. PDC EHS Professional Tony Long and Well Tender Carl Caplinger arrived and shut well in. Operator was instructed to removed lines, remove contamination, replace with clean stone and make repairs to lines.

1407920998 2014-08-13 Casing String Witnessed No Violations On the day of my inspection I witnessed the cementing of the 13-3/8 back to surface. Universal was the cementing company used to pump the cement to surface. Total pipe ran- 1,895ft. Diameter of hole was 17.5in. The total amount of cement used for this job was 1,387sks of Gas block 2% Cale. There was 45bbl of cement returns.
