0009732447 2017-03-02 Production Wells No Violations On the day of my inspection following up on Complaint ID 5522, I witnessed no visible gas leak was present near the wellhead and the well was equipped with a plunger lift system and the valves were in the open position. Gauges present read 800 pounds per square inch (psi) on the casing and 400 psi on the tubing with annular pressure monitoring in place. The production equipment included 16-500 barrel tanks within containment and shared with the 1H, 3H and 4H. Also present, a GP Unit, a 2nd stage separator, a vapor recovery system, a vapor recovery system, a compressor unit and 2-combustors.
0067892462 2018-05-17 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with production valves in the open position. Gauges were present reading 270 pounds per square inch on the tubing, 678 pounds per square inch on the production casing, 150 pounds per square inch on the 13.375 inch diameter intermediate, and 0 pounds per square inch on the 9.625 inch diameter second intermediate casing. The 13.375 inch diameter intermediate casing was plumbed into the same production line as the production casing and tubing. The production equipment consisted of sixteen 500 barrel tank, four vapor recovery towers, four gas production units, one gas conditioning skid, one heater treater, one compressor, one vent scrubber, two combustors, and a Triethylene Glycol dehydration system consisting of one glycol contact tower and reboiler.

1485877960 2017-01-31 Production Wells No Violations On the day of my inspection following up on Complaint ID 5522 and accompanied by ODNR Engineering Tech Wayne Cottle, we met with landowner Mike Cole and reviewed his list of complaints. We completed walk around determining if any encroachment off of well site boundaries had occurred, observed installed discharge drains for surface water, observed sound barrier which has been installed around a new compressor unit and light bubbling coming from the cellar on the said well. The well was equipped with a plunger lift system and the valves were in the open position. Gauges present read 850 pounds per square inch on the casing and 460 pounds per square inch on the tubing with annular pressure monitoring in place. The production equipment included 16-500 barrel tanks within containment and shared with the 1H, 3H and 4H. Also present a GP Unit, a 2nd stage separator, a vapor recovery system, 2-combustors and the newly installed compressor unit. Utilizing an ODNR Honeywell PhD6 meter revealed LEL levels at 0. I placed call to PDC EHS Supervisor Maureen Kertis stating my findings, requesting that they determine where the bubbling is coming from and make necessary repairs. Ms. Kertes stated that a hospital muffler will also be installed on the compressor unit upon receipt.
1520680408 2015-04-06 Completion Testing No Violations On the day of my inspection accompanied by ODNR Inspector Matt Fulk, I witnessed that Mountain States Pressure Services & Well Services Rig # 168-03 was replacing rams on the BOP with drilling out the 1st kill plug set at 7080 feet to follow. The 2nd kill plug is set at 7120 feet and the 1st frac plug set at 7980 feet.

1779677126 2014-07-30 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that First Class Drilling Rig #6 had drilled a 24 inch diameter surface hole to the depth of 544 feet and ran 544 feet of 20 inch 94#/ft. casing and had to wash down the last 5 feet . Universal Well Services was on site and gained circulation with 25bbls of gel, 5bbls of fresh water and cemented casing with 706 sacks of Type 1 cement with 3% CaCl and .25#/sk. of cello flake. While dropping the plug they believed it kicked sideways so the cement was displaced with 183bbls of fresh water. Had good cement returns of 35bbls to surface and no pressure test was performed. Cement head was shut in and they were instructed to leave shut in for 8 hours and complete pressure test.
